Best Tirzepatide for PCOS in 2026 Ranked & Reviewed

Insulin resistance drives much of polycystic ovary syndrome, which is why a GLP-1 often moves weight and metabolic markers that diet alone does not (SURMOUNT-1 reached up to ~21% average body-weight loss). There is no PCOS indication behind that. Tirzepatide is prescribed off-label here, sometimes alongside metformin and sometimes in place of it, and off-label prescribing is legal when a licensed clinician judges it appropriate. The table below scores sellers on price, disclosure and licensure. It cannot tell you whether a given clinic treats PCOS as a reason to prescribe, so raise it during intake.

Methodology & sources

Each provider here carries a score from our published six-factor rubric[1], and the list is then cut down to the criteria named in the page title.

Published Phase 3 efficacy for tirzepatide (~20.9% mean weight loss at the 15 mg dose over 72 weeks) comes from the SURMOUNT-1 trial[2]. The SURMOUNT-5 head-to-head published in 2025 directly compared tirzepatide and semaglutide[3].

Best for: semaglutide at $99/month, 48% under the register median

9.1

Billed in 3-month blocks, the compounded GLP-1 semaglutide starts at $99/mo and compounded tirzepatide at $183/mo. Dr. Patrick Taylor, MD founded the practice and every plan is written by a doctor, filled at a licensed US pharmacy and delivered to the door. Messaging the care team has no cap and nothing locks you in. Peptide and hormone therapy round out the menu. Telehealth reaches all 50 states.

Gala

Verified partner

Best for: moving between compounded and brand without changing seller

8.8

A yearly subscription is the format: compounded semaglutide and compounded tirzepatide are sold that way, with a cheaper microdosing program beside them and brand-name Ozempic on the same menu. The corporate name is AI Coaching Inc., which does business as Gala GLP-1 and is incorporated in Delaware. Providers licensed in every one of the 50 states, plus shipping at no cost, are what the marketing site claims. A Wegovy pill taken orally is flagged as coming soon.

Telos Rx

Verified partner

Best for: starting below a standard dose, with microdose tiers

8.3

Read the $49 headline at Telos Rx carefully: it is a first-month intro rate and the standing cost sits above it. Superior Health Solutions LLC operates the platform, which sells GLP-1 weight loss beside longevity, recovery and sexual-health peptides. Compounded semaglutide and tirzepatide come as injections, or as needle-free sublinguals, with a microdosed tirzepatide too. Providers adjust doses, labs are monitored, delivery is free and discreet, FSA/HSA works and canceling is open. Compounded drugs are not FDA-approved. No pharmacy, no state list, no LegitScript.

$199 and $299 a month are the averages on the compounded plans, with messaging, check-ins and provider care folded in, no membership charged on the side, and the figure unchanged at every dose. Mounjaro, Zepbound, Ozempic and Wegovy sell under their brand names next to compounded tirzepatide and semaglutide. Brentmoor, Inc. is the operator. Disclosure is unusually thorough here: NPIs you can verify against two named advising physicians, Lion MD as clinical partner, six pharmacy partners named, and all 50 states plus DC covered.

HealthRX

Verified partner

Best for: compounded semaglutide at $133/month

7.8

Three things are missing from the site: the name of the pharmacy doing the compounding, the roster of states where prescriptions are written, and any medical director. What is there is a LegitScript certificate, number 50087439, and the FDA's compounded-drug disclaimer. Prices hold steady whatever your dose, insurance plays no part, and overnight cold shipping costs nothing extra. A clinician licensed in the US turns around the brief online assessment inside 24 hours; if the answer is yes, a 503A compounder holding a license sends the semaglutide or tirzepatide within 24-48 hours.

HumeCare+

Verified partner

Best for: an oral route if you will not self-inject

5.7

Plans open from $199/mo, billed month by month and cancellable whenever, and both the drug and the hardware sit inside that. HumeCare+ comes from Hume Health, the company that sells body-composition scales, and the logic follows: clinician-prescribed compounded GLP-1 semaglutide arrives with a Hume Pod, so the patient can watch whether the weight leaving is fat or muscle. Which states it serves is unpublished, and the marketing site identifies neither the prescribing medical group nor the pharmacy.

NextMeds

Membership is a single flat monthly charge, and inside it sit free shipping, a registered dietitian holding a license, a care coach working 1:1, telehealth visits and the drug itself where one is prescribed. Insurance is not billed. A free assessment online goes to a licensed provider, who adds a phone or video consult in states that demand one, then prescribes if it fits. Compounding pharmacies within the Next Meds network, all licensed, ship. Auto-renewal does not exist, cancellation is open, and all 50 states can order compounded tirzepatide and semaglutide.

That single figure absorbs the drug, unlimited 24/7 provider messaging, and delivery. Physicians hold licenses in all 50 states, and Strive Pharmacy, a 503A compounder, fills the orders.

Bon Health

Best for: semaglutide at $170/month, 36% under the register median

8.5

How long you commit sets the rate at Bon Health: $330 month-to-month at the top, falling to $170/mo if you sign for a year, on compounded GLP-1, semaglutide or tirzepatide. Membership itself costs $0 and billing can be canceled whenever. A comprehensive physician review sits behind each order, dosing is titrated by that physician, free 24/7 physician support is included, and parcels land in 1-3 days. Licensure spans all 50 states. Wegovy, Ozempic, Zepbound and Mounjaro can be had under their brand names too. The practice is physician-led.

Enhance MD

Best for: knowing which pharmacy fills the vial — it names Tru Meds Rx

8.5

$112 buys the first month on Core, once $100 comes off the opening order with code SUMMER100. Core is the semaglutide tier, Advanced is tirzepatide, Elite combines the two. Before any of that, Enhance MD requires two mandatory steps: baseline metabolic labs, drawn at Quest Diagnostics, then a live video visit with a board-certified clinician. Most sellers in compounded GLP-1s onboard asynchronously and ask for neither, so this is the more rigorous route in.

Trellis Vitality

Best for: knowing which pharmacy fills the vial — it names VialsRX

8.5

$150 a month opens compounded semaglutide and $199 compounded tirzepatide, identical at every dose, with no consult fee or membership stacked on top. Scale weight is not the frame; body composition is, which is why biomarker panels taken at home, a re-test at 90 days, and muscle-sparing choices such as glycine formulations and tesamorelin come with the GLP-1s. Trellis does the coordinating only: VialsRX in Houston dispenses and the Arora Health provider network prescribes.

Luma Meds

Best for: semaglutide at $140/month, 38% under the register median

8.0

Nothing here happens in a clinic; the whole thing runs online. The GLP-1 pair, semaglutide and tirzepatide, arrives as injections, compounded at 503A and 503B pharmacies the site describes as FDA-regulated. A licensed physician oversees care. Delivery is free and discreet, support runs 24/7 with no cap, and Luma states that no fees are hidden. It holds LegitScript certification.

Mint Med

Best for: semaglutide at $117/month, 40% under the register median

8.0

The price is $159/mo in 36 states, flat across all doses, so patients do not hit the per-tier escalation common as they titrate up; a promo code, 175NOW, cuts the first six weeks to $175. Mint Med is a national compounded-GLP-1 telehealth platform with no membership, selling semaglutide and tirzepatide. Patients can cancel anytime on 48 hours' notice, it is HSA/FSA eligible, and it is LegitScript Certified. Mint Medical Clinic, which is Utah-only, is a different company.

NuviaRx

Best for: semaglutide at $140/month, 36% under the register median

8.0

Compounded GLP-1 opens at $140/mo for semaglutide and $170/mo for tirzepatide, delivery included. A clinical evaluation, the prescription where you qualify, and the drug itself from a partner pharmacy all sit inside that figure with nothing added later. NuviaRx publishes a complete legal suite and identifies itself as NuviaRx Health Inc., trading as NuviaRx Health Partners out of Jacksonville, FL, though California law governs. Cash pays for everything; insurance is absent. Monthly rates are flat, shipping is free, and 47 states are covered. Rush Pharmacy fills the prescriptions, a 503A compounder the company calls PCAB-accredited. Injections of B12/MIC and NAD+ are also on offer from Teleclinic Holdings LLC, whose compounded tirzepatide and semaglutide are prescribed by physicians. LegitScript certifies it. CeliaRx

Best for: semaglutide at $125/month, 34% under the register median

7.9

Tablets begin near $199/month; injectable vials average as little as $125-$225/month. Screening by a clinician happens once you have checked out, and the money comes back in full if approval does not follow. Partell Pharmacy compounds what is prescribed. The marketplace leans toward longevity, listing a wider peptide catalog for brain, gut, hair and skin beside compounded tirzepatide and semaglutide sold as B12-combo injectables or dissolving oral tablets.

EdgeRx

Nothing can be bought yet. Prices and descriptions for both GLP-1 injections are up, but the protocols sit behind a waitlist and checkout has not opened for any of them. On the 52-week plan compounded semaglutide starts at $199/mo and tirzepatide at $269/mo, the same figure every month, nothing hidden, cancellable at will. REMEVi claims to be the first GLP-1 telehealth clinic working bilingually in English and Spanish, with clinicians trained in both rather than anything auto-translated. Providers manage titration weekly and an NPI-verified pharmacy ships overnight. Certification is LegitScript and licensure covers all 50 states plus DC. Trinity Meds

7.7

Trinity Meds charges an all-inclusive flat rate of $149/month for compounded semaglutide and $199/month for tirzepatide, the same price at every dose, with free shipping and no insurance; multi-month plans drop the price further. Try Ageless

Best for: semaglutide at $119/month, 37% under the register median

Semaglutide by injection opens at $119/month, about as low an entry price as we track anywhere, and aggressive introductory pricing runs across all five products. Three delivery formats exist for the compounded semaglutide and tirzepatide sold here: injection, troche, and drops placed under the tongue. Nobody else in our directory spans that many. The platform frames itself around longevity and metabolic health, and holds LegitScript certification.

Commit longer and pay less: semaglutide spans $99-$175/mo and tirzepatide $135-$225/mo, dose making no difference, and each tier absorbs expedited shipping at no cost, the drug, the prescription and the consultation, with membership charged to nobody. Try Curve LLC leans on providers from Beluga Health, licensed everywhere in the 50 states, and on a 503A compounder in the US, VialsRx. BioGlow Health

Best for: tirzepatide at $169/month, 37% under the register median

7.5

Two omissions stand out: BioGlow names neither its clinicians nor the pharmacy behind its compounding. Its prices start above where most compounded competitors begin. The brand runs several verticals under physician supervision, weight loss, peptides, testosterone, ED and hair, shipping compounded drugs to all 50 states with the consultation and the delivery both free. For weight it markets compounded semaglutide, tirzepatide and retatrutide, ordered through an online intake a provider then reviews. Retatrutide remains an investigational molecule that the FDA has not approved.

Carocare Wellness

Best for: compounded semaglutide at $170/month

7.5

Month one at Carocare Wellness costs $170 for the compounded GLP-1 semaglutide and $200 for tirzepatide, then climbs as the dose escalates, which makes the 3-month bundle the cheaper road: $500 for semaglutide, $600 for tirzepatide. Provider support, supplies and delivery ride along in every package. This is an in-person clinic in Houston, Texas, medically guided and supervised by a provider, that also does IV hydration, NAD+ and routine wellness visits. Caroline runs it, serving the Houston metro, and consultations get booked through Calendly rather than any national mail-order portal.

Intimate Rose

Best for: semaglutide at $169/month, 37% under the register median

7.5

169 dollars per month is the published floor for oral compounded tirzepatide or semaglutide, taking in delivery at no charge and the drug, with continuing provider support and neither contract nor membership attached; the weight-loss page states this openly. Long established in pelvic and women's health, the brand added this online weight-management program later. Compounding pharmacies and licensed providers in the OpenLoop Health network handle fulfillment and clinical work. Payment is cash only, and FSA and HSA both qualify. FDA approval does not extend to compounded GLP-1s.

Aurelius Health Group

Best for: starting below a standard dose, with microdose tiers

FitFlow Health writes the two GLP-1s as 'semaglutide (=Wegovy)' and 'tirzepatide (=Mounjaro)', an equivalence it asserts itself. The platform is 100% online: a 4-step process reaches a board-certified provider, then US-licensed compounding pharmacies fill the order. 7.3

Jyra Health describes the rates as limited time: $99/mo is where the compounded GLP-1 semaglutide starts and $133/mo where tirzepatide does, so treat both as promotional. Insurance is not involved, membership is not charged monthly, no fees hide anywhere, and express delivery is free. Board-certified providers prescribe across all 50 states. Privacy, Terms, HIPAA and Refunds pages are all published. The company discloses compounded drugs are not FDA-approved. TrimRx

Best for: compounded semaglutide at $179/month

Three things TrimRx does not publish: the pharmacy that compounds its drugs, the states it serves, and any LegitScript certification. Specifics sit behind an intake quiz. What is visible is a cash-pay program with tiered monthly pricing plus multi-month bundles, no insurance, selling compounded semaglutide and tirzepatide shipped monthly at no shipping cost. Licensed US providers prescribe after reviewing an online medical history. The pitch is dose continuity and stable pricing: "switch and keep your current dose, no price hikes". Around that funnel sits a heavy body of educational content and a directory, hosted on the same site, comparing rival providers. Physicians supervise, and orders above $150 ship free.

Both are compounded GLP-1, drawn from 503B outsourcing facilities it describes as FDA-registered, and released after a quiz plus review by a licensed provider. Every 4 weeks is the billing rhythm, not monthly, and dose does not alter the flat charge, which starts at $249 per 4 weeks for semaglutide with shipping, the drug and the consultation folded in. An online medical intake reaches a clinician holding a license, who decides. AZ Health

Best for: seeing what it charges before you answer any medical questions

6.6

Flat monthly pricing at AZ Health puts physician-prescribed compounded semaglutide at $179/mo and tirzepatide at $199/mo, insurance not required; NAD+ injections are sold too. FSA/HSA money works, delivery is free and discreet, and canceling is unrestricted. The clinic is Arizona-based and 100% virtual. It states that compounded drugs are not FDA-approved and need a prescription. The whole site is a single page carrying no Terms or Privacy, and the LegitScript certification it claims is self-stated with no way to verify it. AZHM is the shorthand it uses.

BloomWell Rx

Best for: seeing what it charges before you answer any medical questions

6.6

A free visit precedes pricing that begins at $199/mo for personalized compounded semaglutide and $249/mo for tirzepatide, marketed as GLP-1+GIP. NAD+, sermorelin and glutathione are sold beside them, and the service is 100% online. BloomWell says plainly that it is neither a pharmacy nor a manufacturer: a licensed third party compounds and dispenses. A complete legal suite is up. That third party is never named, no LegitScript certification appears, and a marketing agency operates the site with the medical entity left unclear.

Compounded GLP-1 semaglutide opens at $199/month, though the glycine and B12 formulas run $229; tirzepatide begins at $289/month. Code WELCOME100 saves a new patient $100. Delivery costs nothing and insurance is not needed. A short questionnaire and a clinician review come first. EOS is unusually forthcoming for the category, putting a name to both its clinical partner, Arora Health & Aesthetics, and its pharmacy partner, Jungle Jim's, and its footprint matches the 40 states that pharmacy is licensed in. Best for: compounded semaglutide at $249/month

6.3

Prices are banded by dose and posted publicly. The semaglutide-range plan costs $249-$349/month; the tirzepatide-range plan, a GLP-1/GIP paired with B12, runs $349-$649/month. Both bands include the medication, and shipping is free once a prescription is issued. Elara MD sells weekly injectable GLP-1 therapy for weight loss and wellness, and its peptide menu covers glutathione, NAD+ and sermorelin. Marketing claims reach over 40 states. Not to be confused with Elara Health and Wellness, a separate company.

Green Cap Health

Best for: seeing what it charges before you answer any medical questions

6.3

The GLP-1 weight-loss injection opens at $79 for month one. Green Cap Health works only in Texas and owns both ends of the transaction: it is the clinical practice writing the prescription and the compounding pharmacy filling it, so no outside partner touches the order. Hair, skincare and sexual-wellness products share the shelf with compounded semaglutide.

LifeRx.md

Best for: coverage in all 50 states, which most of the register will not confirm

6.3

The dose can climb without the price moving. A five-week supply of the compounded GLP-1 semaglutide costs about $200 and compounded tirzepatide about $275, flat. Around those sit roughly $30 of shipping on each order, a $49.95 first consult and $25 follow-ups, putting the real monthly figure near $255. State-licensed 503A pharmacies do the filling and a video consult is required to start. LifeRx.md carries LegitScript certification, reaches all 50 states, and takes HSA and FSA.

Loop Health

Best for: coaching bundled into the price rather than sold separately

6.3

Membership is the product, not the drug. $79/month is the entry, or $1,500/year, which carries a $180/month research credit toward compounds; the pharmacy bills for each medication on its own. Compounded semaglutide and compounded tirzepatide sit inside the Loop Lean program, with peptides, NAD+, sermorelin, coaching and bloodwork around them. This US platform introduces members to independent licensed providers instead of writing prescriptions itself.

Orders are pay-per-order: no subscription, no sign-up fees, and every order ships free. Revive Meds (revive-meds.com, Revive Health Therapy) serves all 50 states, is HSA/FSA eligible, and fulfills through named 503A compounding pharmacies. Longevity care sits alongside the GLP-1 line.

Remi Meds

Best for: compounded semaglutide at $369/month

5.7

Every dose tier carries a published price instead of a floor-only 'as low as' figure: semaglutide $249-$369/mo, tirzepatide $349-$639/mo. Remi Meds is a DTC platform selling compounded semaglutide and tirzepatide-with-B12 by tier, with Glutathione, Sermorelin and NAD+ sold as add-ons. Both its clinical network, Beluga Health, and its pharmacy partner, Strive Pharmacy of Gilbert, AZ, are named. Remedy Meds is a separate company.

Royal Medical Center

Best for: bloodwork inside the $650/month rather than billed on top

5.7

Compounded tirzepatide is priced along four dose tiers, $450 through $1,100/mo, each covering drug, supplies, labs and consultations. Semaglutide is not offered at all. Dr. Dagoberto Rodriguez, M.D. founded the practice, which works nationally by telehealth and treats hormone therapy as its core; the credentialing and the itemization are both unusual for this segment. Gaya Wellness

Best for: bloodwork inside the $349/month rather than billed on top

Gaya Wellness runs three tiers and only the upper two include drugs: $149/mo buys GLP-1 access with no medication attached, $349/mo adds the compounded medication, and $549/mo puts GLP-1 together with HRT. Board-certified physicians write compounded semaglutide or tirzepatide, backed by a lab panel of 50+ biomarkers, weekly asynchronous check-ins and payment that qualifies for HSA/FSA. The practice is virtual and OB/GYN-led, aimed at women 40+. You must be 18-64 with a BMI of 30+, or 27+ carrying a metabolic condition. Licenses cover FL, NC, VA, IN and TN.

Member prices are $109/month for compounded semaglutide and $159/month for compounded tirzepatide, but reaching them means a three-month minimum commitment, a one-time $99 consult and a $129/month membership on top. Vita Bella Health, LLC runs the Nevada-based platform, which puts anti-aging care, sexual-wellness, peptides and hormone therapy beside the compounded GLP-1s. Luci

Luci competes on price for compounded GLP-1: $99/month at the bottom for compounded semaglutide, $145/month for compounded tirzepatide, with pill kits of both molecules available orally. An independent US telehealth medical group called DrTelx handles the clinical side, reading a short questionnaire before a provider signs off, then staying on for ongoing support. Neither compounded molecule holds FDA approval for weight loss. Only select US states are served and Luci never prints the full list. The pharmacy goes unnamed.

Shape Vantage

Best for: compounded tirzepatide at $599/month

5.5

One product, priced at $599, is all Shape Vantage sells: tirzepatide, bought through a direct checkout link with no medical intake visible anywhere in the path. Its terms leave provider review as something that may happen rather than something that must. The profile is a hybrid gray-market one and does not fit how compounded telehealth normally works. GLP Watchdog does not recommend Shape Vantage. Establish what the actual prescription pathway is before any money changes hands.

Evernow

4.7

Membership at Evernow starts around $35/month for menopause access, and medication cost depends on insurance. The menopause-centered women's platform is now accepting insurance for video visits, listing Blue Cross Blue Shield, Anthem, United Healthcare and Aetna, and offers 24/7 clinician messaging on one-time or membership care.
