Below are 16 we checked ourselves against primary sources — benefits portals, vendor partnership pages, OPM filings, plan documents — with the source link and a direct quotation on every entry. Not finding yours here is common; the five steps further down work on any plan and take under ten minutes.

Getting the answer for your own plan, in five steps

Five steps, roughly ten minutes, and they work on any employer-sponsored plan in the US. What you end up holding is the answer for your own plan year, which is the only answer that counts.

  1. 1

    Log into your benefits portal

    It will be BenefitFocus, Workday Benefits, ADP, BSwift, Empyrean, Mercer Marketplace, Businessolver, or something your employer built — and HR can tell you which. Everything you need is in there: the Summary of Benefits and Coverage, the Summary Plan Description, and the prescription drug formulary.

  2. 2

    Download the prescription drug formulary

    Look for a PDF labeled “Formulary,” “Drug List,” or “Pharmacy Benefit Summary” for the current plan year. It will list every covered drug with its tier (1, 2, 3, 4) and any prior authorization or step therapy requirements. If your plan uses a separate Pharmacy Benefit Manager (Caremark, ESI, OptumRx, MedImpact, Prime, Navitus, Costco PBM), the formulary may be on the PBM's site instead.

    Search the formulary for these exact terms

    • “Wegovy” (semaglutide for weight management)
    • “Zepbound” (tirzepatide for weight management)
    • “semaglutide”
    • “tirzepatide”
    • “orforglipron” or “Foundayo” (the new oral GLP-1)
    • “weight management drugs” or “anti-obesity drugs”
    • “exclusion” (some plans list weight-loss drugs as a category-level exclusion)

    Find either drug and the thing to read next is its tier, along with any note about prior authorization or step therapy. Find neither, and your plan almost certainly excludes them from its weight-management benefit altogether.

    Cross-reference with the plan administrator's clinical policy bulletin

    Even if your formulary lists Wegovy, the actual approval criteria live in the plan administrator's clinical policy bulletin. Aetna publishes CPB 0040, Cigna publishes coverage policies under their medical necessity manual, UnitedHealthcare publishes its “Pharmacy Coverage Determination Guidelines,” and BCBS plans publish per-state medical policies. These documents tell you the specific BMI thresholds, comorbidity requirements, and supervised-weight-management prerequisites that apply.

    Call member services if anything is unclear

    Your insurance card carries the number. Ask it plainly: is Wegovy covered for chronic weight management on this plan, and what does prior authorization require? They have to answer. Get it in writing where you can — a chat transcript or an email is what protects you if the story changes later.

Reference benchmarks, not authoritative for your plan

Treat those 16 entries as benchmarks and nothing firmer. One employer can cover a drug on one plan tier and not another, differ between union and non-union staff, differ by state, differ once a deductible is met, and differ again depending on which PBM runs the pharmacy benefit. Anti-obesity coverage also moves mid-year, as plans add drugs, drop them, or tighten the conditions. Only one document settles your case: the formulary for your plan year, on your portal. The five steps above get you to it.

How to read this tool

Every card covers the same four drugs. Wegovy and Zepbound are where the decisions move around, because both are approved specifically for weight loss and neither counts as a diabetes medication. Ozempic and Mounjaro are approved for type 2 diabetes, and coverage runs wider for that reason — a chronic disease is easier for a plan to say yes to — though prior authorization is still almost always in the way.

Coverage status definitions

  • Covered — on the formulary at ordinary cost-sharing. You pay whatever copay or coinsurance that tier carries.
  • Prior auth — on the formulary, but nothing gets filled until your prescriber files a request setting out your BMI, your comorbidities, and what you have already tried for weight.
  • Step therapy — before a GLP-1 is approved you have to have tried something cheaper and had it not work. Usually phentermine, or one of the older anti-obesity drugs.
  • Not covered — off the formulary altogether. It is cash price, or a telehealth cash-pay route.
  • Unknown — the employer has said nothing publicly, and no patient report has reached us that we could check. Read it as a reason to call the benefits line.

Confidence labels

Each card also says how far to trust what it claims:

  • Verified from plan documents — read straight out of a published brochure or formulary.
  • Reported / publicly discussed — the employer has said something about it, in the press, on an earnings call or to its own staff, and we have not matched that against the live formulary.
  • Unverified — patient must confirm — No authority behind it. Coverage defaults to Unknown, and the card exists so that someone searching for the employer meets an honest “we do not know, and here is how to find out” instead of a guess dressed up as an answer.

Methodology

The 16 employers in this tool were selected based on US headcount and search volume for “does [employer] cover Wegovy.” Most employers in the United States do not publicly disclose their pharmacy benefit formularies — that information lives behind a benefits-portal login. Where an employer has been quoted in trade press or earnings calls discussing its anti-obesity drug coverage, we mark the confidence as “reported.” Where coverage is not publicly disclosed, the default is “Unknown” with confidence “unverified.” We deliberately do not guess. The value of this tool is the structured framework and the verification prompt, not invented coverage data.

What to do next

Finding your employer here is a place to start, not a finish. Call the benefits line or open the HR portal, and go looking for the prescription drug formulary attached to the plan year you are actually enrolled in. Search it for semaglutide, tirzepatide, Wegovy and Zepbound. Their absence is itself the answer: the weight-loss indication is almost certainly not covered.

When the answer comes back no

Two routes remain open:

  • Manufacturer self-pay programs. Novo Nordisk and Eli Lilly each sell direct to patients for cash, Wegovy and Zepbound respectively, well below list.
  • Telehealth sellers. A great many will write for a GLP-1, brand or compounded, against cash. Our provider comparison for an independent ranking.
