Foundayo Cost: Orforglipron Price by Dose, Insurance and Medicare

Foundayo (orforglipron) is $149 to $349 a month self-pay from LillyDirect, by dose, and $299 at maintenance with on-time refills. What a 72-week course costs, the savings card, Medicare's $50 Bridge copay, and seller fees.

Part of What it costs, and who pays

Without insurance, Foundayo costs $149 to $349 a month from LillyDirect, Eli Lilly’s own pharmacy, depending on the dose. The two top doses drop to $299 when each refill is bought within 45 days, so maintenance costs $299 a month with refills on time.[1][2] Commercial insurance that covers it, plus Lilly’s savings card, brings a fill to as little as $25. Medicare’s GLP-1 Bridge charges $50 a month.[2][5] Orforglipron has no other price: Foundayo is the only orforglipron product approved in the United States.[7]

About this article. Lilly’s prices and savings-card rules are as published on LillyDirect and in the Foundayo terms in October 2026; Medicare terms from Medicare.gov and CMS; dose steps and trial results from the FDA label. Seller figures are drawn from the GLP Watchdog register, each dated to the month its pricing was last checked. The full clinical picture is on the Foundayo hub. Prices change; confirm at checkout.

Foundayo price at each dose

LillyDirect self-pay price per 30-day bottle, October 2026. Each bottle holds 30 tablets.
DosePrice nowFrom January 1, 2027Condition
0.8 mg (start)$149$149Regular price
2.5 mg$149$199Temporary cut ends December 31, 2026
5.5 mg$199$299Temporary cut ends December 31, 2026
9 mg$299$299Regular price
14.5 mg$299$299Refill within 45 days, or $349
17.2 mg$299$299Refill within 45 days, or $349

The third column assumes Lilly lets the temporary prices lapse on schedule; it can extend, change or end any of these offers without notice.[2] Every strength comes as a bottle of 30 tablets, and Lilly defines a month as 30 days, so one bottle is one month.[1][6] At $299, a day of Foundayo costs about $9.97.

The 45-day rule is the one that costs money. The $299 price on 14.5 and 17.2 mg is a purchase offer that applies automatically on the first fill and stays only while each refill is completed within 45 days of the last delivery. Miss that window and the month is $349.[2] A longer gap has a second cost: the label says that after 7 or more missed doses in a row, the dose is restarted lower and climbed again, which means buying lower-strength bottles a second time.[6]

What a course of Foundayo costs

The label starts everyone at 0.8 mg and moves up no faster than every 30 days, to 2.5 mg, then 5.5 mg, then on to 9, 14.5 or 17.2 mg depending on response and tolerance.[6] That makes the bill predictable. Climbing to 17.2 mg at the fastest pace, at the regular prices that apply from January and with every top-dose refill on time:

  • The first 12 bottles (360 days) cost $3,338: $149, $199, then ten bottles at $299.
  • A year at maintenance on 9, 14.5 or 17.2 mg is about $3,638.
  • A 72-week course, the length of the trials, takes 17 bottles and costs $4,833. If every top-dose refill came late, the same course would cost $5,483.

Bottles bought before January 1, 2027 cost less. Someone starting in November 2026 pays $149 for 0.8 mg and $149 for 2.5 mg in December, then moves to 5.5 mg in January at $299 rather than $199. The saving from the temporary prices is at most $150 per person, and only for people on those two doses this year.

Following its own label’s schedule, the first 12 bottles of the Wegovy pill also cost $3,338 at NovoCare’s self-pay prices. The two pills are priced to the dollar against each other.[8] The Wegovy pill’s own prices, insurance terms and seller fees are on Wegovy pill cost.

Cost per point of weight lost

Because 9 mg and the two top doses share a price, a full course costs about the same whichever maintenance dose a person ends on. The difference is in the result. Foundayo’s label reports weight change after 72 weeks in Trial 1, which enrolled 3,127 adults without type 2 diabetes.[6] Dividing the course cost by those results:

Trial 1 (no diabetes), intent-to-treat change in body weight at week 72, from the Foundayo label. Course cost is 17 bottles on the label's schedule at regular LillyDirect prices with every refill on time.
Maintenance doseWeight change72-week costPer point lostPer point beyond placebo
5.5 mg−7.4%$4,833$653$912 (net −5.3%)
9 mg−8.3%$4,833$582$780 (net −6.2%)
17.2 mg−11.1%$4,833$435$537 (net −9.0%)

The placebo group lost 2.1% on diet and exercise counseling alone, so the last column takes that out.[6] On either measure, the top dose buys the most weight loss per dollar, because the trial’s best result costs nothing extra if refills are on time. Read the table with three limits. These are trial averages, and individuals vary widely. The figures count everyone randomized, including the roughly one in four who stopped the drug, and the label imputed missing weights.[6] And a person only reaches 17.2 mg if they tolerate it; nausea, vomiting and diarrhea all rose with dose.[6] The side effects by dose are covered in Foundayo side effects.

No cost-per-point comparison with other drugs is given here. Foundayo, the Wegovy pill, Wegovy and Zepbound were tested in separate trials of different lengths and designs. Dividing one drug’s price by another trial’s result would produce a number that looks precise and is not.

Foundayo cost with commercial insurance

Lilly’s Foundayo Savings Card works two ways, depending on what the plan does:[2]

  • Plan covers Foundayo. A fill can cost as little as $25, whether it holds one, two or three months. Savings are capped at $100 per month’s supply and $1,000 per calendar year, for up to 10 fills a year.[2]
  • Plan does not cover it. The card prices it at the self-pay levels: $149 at 0.8 mg, $199 at 5.5 mg for now, $299 at 9 mg and above.[2]
  • Government coverage is excluded. Medicare in any form, Medicaid, Medigap, VA, DoD and TRICARE beneficiaries cannot use either version.[2]
  • Both versions expire December 31, 2026. Lilly has not said what replaces them.[2]

The $1,000 annual cap is the number to check. With a $125 copay, the card takes off $100 and you pay $25, which uses the whole cap in 10 months. With a copay of $200, you pay $100 a month and reach the cap at the same point. Under the card’s terms, each tier’s price plus its maximum saving comes to $669 a month at every dose. Lilly does not publish a list price for Foundayo on these pages.[2] Plans that run an alternate funding program are barred from the card altogether.[2]

Lilly lists Foundayo as FSA and HSA eligible, with eligibility depending on the plan, so pre-tax money can pay the self-pay price.[1][3]

Foundayo cost with Medicare or Medicaid

Since July 1, 2026, Medicare covers Foundayo for weight management through the Medicare GLP-1 Bridge, a temporary CMS program that runs through December 31, 2027. The copay is $50 for a one-month supply.[5][9] CMS updated its program guidance on April 6, 2026, to add Foundayo after the FDA approval.[9]

  • Eligibility starts with a Part D drug plan and a BMI that meets the program’s bar at the start of treatment: 35 or more, or a lower figure paired with specific conditions.[5]
  • It is for weight management only. A prescription written for type 2 diabetes, for sleep apnea or for a fatty liver does not qualify, and neither does anyone whose Part D plan already pays for their GLP-1.[4][5]
  • Your prescriber completes a prior authorization once. That approval carries over to refills and dose changes until the program ends on December 31, 2027, as long as you stay on the same drug.[5]
  • The $50 sits outside Part D: it is not credited to the plan’s deductible or its yearly out-of-pocket cap, and Extra Help cannot reduce it.[5]

Against self-pay, the Bridge saves $249 a month at maintenance. Who qualifies, and how to apply, is in the Medicare GLP-1 Bridge. Lilly’s cards exclude Medicaid.[2] Whether a Medicaid plan pays for Foundayo is up to that state’s program; check yours with the state Medicaid GLP-1 checker. For people with no coverage and low income, help programs are compared in GLP-1 patient assistance programs.

Foundayo price through a telehealth seller

Telehealth sellers prescribe Foundayo, and several records note that LillyDirect fills the prescription, so the tablets cost what Lilly charges and the seller adds its own fee. These are the 20 sellers in the GLP Watchdog register with a published price for the approved product. Of them, 14 list Lilly’s own $149 starting price and 6 list more. Where the record gives a recurring monthly fee, a month at the starting dose comes to $198 to $298 all in.

Foundayo prices published by sellers in the GLP Watchdog register. "All in" adds the recurring monthly fee to the lowest price where the record states one. The last column is the month the seller's pricing was last checked.

AskRx ($100 a month), Bloom MD ($75 a month), Kin Meds ($149 a month) and Tummala Health ($99 a month) publish only their own fee; the tablets are bought separately, at LillyDirect’s price for the dose. NowPatient ($133.85 delivered, starting dose) ships Foundayo from a pharmacy outside the United States; read the review for what that means for the product and the paperwork.

What the fee buys is the prescription and the follow-up. Someone who already has a prescriber can send the prescription straight to LillyDirect and pay only Lilly’s price; LillyDirect fills through Amazon Pharmacy, Fuze Health, Gifthealth and Walmart Pharmacy, with free delivery or pickup.[1] That channel has its own page, LillyDirect Foundayo: it fills a valid US prescription from any prescriber, and for anyone without one it can point to independent telehealth doctors. Sellers are ranked on more than price at the best Foundayo providers, and prices across every GLP-1 are tracked on the price tracker.

“Compounded orforglipron” is not Foundayo. Lilly holds the only FDA approval for orforglipron, and Foundayo is the only product under it.[7] A tablet sold under the molecule’s name by anyone else is not the approved drug. In the register, Bayan Health lists a product described as compounded orforglipron. It is not Foundayo and is not in the price table.

Foundayo vs. the Wegovy pill, Wegovy and Zepbound per month

Manufacturer self-pay prices at maintenance doses, October 2026. Foundayo and the Wegovy pill are sold in 30-day bottles; a Wegovy box of 4 pens and a Zepbound KwikPen each cover 28 days, so the yearly column adjusts for days.
Drug and maintenance doseSelf-pay per monthDays per fillAbout per year
Foundayo 9, 14.5 or 17.2 mg, refills on time$29930$3,638
Foundayo 14.5 or 17.2 mg, refill late$34930$4,246
Wegovy pill 25 mg$29930$3,638
Wegovy pen 2.4 mg$34928$4,549
Wegovy HD pen 7.2 mg$39928$5,201
Zepbound KwikPen 7.5 to 15 mg, refills on time$44928$5,853
Zepbound KwikPen 10 to 15 mg, refill late$69928$9,112

At maintenance, Foundayo and the Wegovy pill are the cheapest drugs in the table, at $299 a month each.[1][8] The Wegovy pen is $50 more per fill and covers two fewer days. Zepbound at its higher doses is $150 more per fill with refills on time, and $499 at 7.5 mg or $699 above it when a refill is late; Lilly sells the Zepbound vial at the same monthly prices as the KwikPen.[10][11] Both Lilly drugs carry the same 45-day refill condition at the top of the ladder.[2][10] For every route to a lower price across brands and compounded drugs, see the cheapest GLP-1 without insurance.

Anyone still choosing between Foundayo and a Wegovy pen may want a seller that handles brand semaglutide as well. altRX lists brand-name Ozempic and Wegovy, and Aquivia Health lists branded Ozempic by the 4-week fill next to its compounded plans. Measured and MEDGm charge a membership for brand-name care and bill the Wegovy or Ozempic itself separately.

With a commercial plan, the coverage question comes first. Roen Rx, FitRx and AM Rx offer insurance coordination for brand Wegovy and Ozempic, and each charges a monthly membership for that help.

Frequently Asked Questions

At LillyDirect in October 2026, $149 a month at the 0.8 mg starting dose, $149 at 2.5 mg and $199 at 5.5 mg until December 31, 2026, $299 at 9 mg, and $299 at 14.5 or 17.2 mg if each refill is bought within 45 days, otherwise $349. From January 2027 the regular prices for 2.5 mg and 5.5 mg are $199 and $299.
Orforglipron is sold in the US only as Foundayo, so its price is Foundayo's: $149 to $349 a month self-pay by dose, as little as $25 with commercial insurance that covers it, or $50 on Medicare's GLP-1 Bridge. A product sold as compounded orforglipron is not the approved drug.
If a commercial plan covers it, Lilly's savings card can bring each fill, of one, two or three months, down to $25, with savings capped at $100 a month and $1,000 a year, for up to 10 fills a year. The card expires December 31, 2026, and cannot be used with Medicare, Medicaid or other government coverage.
Yes, for weight management, through the Medicare GLP-1 Bridge. Eligible people with Part D pay $50 for each month's supply once a prior authorization is approved. The program runs from July 1, 2026 to December 31, 2027. A prescription for diabetes, sleep apnea or a fatty liver does not qualify.
At maintenance, yes. Foundayo is $299 a month self-pay, the same as the Wegovy pill, against $349 for the Wegovy pen and $449 for Zepbound at 7.5 mg and above with refills on time. Foundayo and the Wegovy pill bottles last 30 days; a Wegovy box of pens or a Zepbound KwikPen lasts 28.
Many sellers in the register list Lilly's own price for the tablets and add a monthly membership or visit fee for the prescription and follow-up. A person who already has a prescriber can send the prescription to LillyDirect directly and pay only the medication price.
