Part of Body changes and sexual health on a GLP-1

No trial has measured what semaglutide or tirzepatide do to the breasts. The changes people notice on Ozempic, Wegovy, Mounjaro or Zepbound track the weight loss, not the drug. Breasts are partly fat, and in women imaged before and after bariatric surgery, average breast volume fell by 39%.[6] None of the four labels lists breast pain or tenderness as a side effect,[1][3] and the FDA’s reporting database holds only a few dozen such reports.[16] On cancer, randomized trials of the drug class show no clear change in breast cancer risk. The one label that mentions breast cancer, Saxenda’s, says its trial cases were too few to tie to the drug.[5]

About this article. Written from the current FDA labels on DailyMed for Ozempic, Wegovy, Mounjaro, Zepbound and Saxenda, studies of breast tissue after surgical and dietary weight loss, randomized and observational cancer data, and FDA adverse-event reports. Breastfeeding is covered separately in GLP-1s while breastfeeding. A new lump, nipple discharge or skin change needs a clinician, whatever you are taking.

Why breasts shrink when weight comes off

A breast is a mix of glandular tissue, which makes milk, connective tissue, and fat. The fat share varies widely between women, and it is the part that responds to weight. Lose body fat and breast fat goes with it. That is the whole mechanism behind what social media calls “Ozempic breasts”; no study has found a direct effect of these drugs on breast tissue.

The best measurements come from bariatric surgery, because many of those patients had a mammogram before the operation and another after. Software can split each image into total breast volume and dense, fibroglandular volume.

Breast tissue before and after weight-loss surgery, measured from mammograms.
StudyWomenBMI before → afterWhat changed
Vohra 20178046.0 → 33.7Breast volume down 39.4%; fibroglandular volume down 15.5%
Hassinger 2019180 (82 with volume data)46.0 → 35.4Total breast volume and fibroglandular volume both fell; follow-up mammogram a median 62 months after surgery

Fat went faster than gland. In the first study, volume dropped by about two-fifths while the dense tissue fell by about a sixth.[6] The second study, which looked at mammograms taken years after surgery, found the same direction.[7] These were people losing large amounts of weight, starting from a BMI near 46. How much of this carries over to the smaller losses typical on a GLP‑1 has not been measured; the direction is the same, and the size is likely to scale with the weight lost, but that is inference.

Shape changes too. Skin that stretched over a larger breast does not shrink back fully, so the breast can sit lower and the two sides can end up less even. A 2026 review written for plastic surgeons lists drooping (ptosis) and asymmetry as changes that can follow drug-induced weight loss, and advises surgeons to counsel patients before any breast surgery.[12] That review is expert opinion with two case examples, the lowest grade of evidence. When skin removal or a lift becomes an option, and why timing matters, is covered in when it is safe to have the skin removed.

Breast pain and tenderness: not on any label

The Ozempic, Wegovy, Mounjaro and Zepbound labels mention breasts only in their breastfeeding sections. None lists breast pain, tenderness, swelling or enlargement among the adverse reactions seen in trials, for semaglutide[1][2] or for tirzepatide.[3][4] The side effects the labels do list are compared in which GLP-1 has the worst side effects. A search of PubMed for any study linking a GLP‑1 to breast pain, tenderness or male breast enlargement (gynecomastia) returned nothing.

What exists is the FDA Adverse Event Reporting System, where patients, clinicians and manufacturers file reports. These are reports, not confirmed side effects. Anyone can file one, nobody checks whether the drug caused the symptom, and the totals reflect how widely a drug is used and how much attention it gets. They cannot give a rate.[16]

A few dozen reports against tens of thousands is not evidence the drugs cause breast pain, and it is not evidence they do not. Breast pain is common in women anyway. The National Cancer Institute notes that fibrocystic breast changes, which affect as many as half of women, cause swelling, discomfort and sensitive nipples that often start before or during a period.[17] Someone who starts a GLP‑1 and then notices tender breasts has many possible explanations, and the drug is only one.

One link is plausible but unproven. A 2026 narrative review on tirzepatide and breast clinics noted that as breast fat and the fat under the skin shrink, lumps that were already there, most of them benign, often become easier to feel.[11] Tissue that was cushioned may also feel different to the touch. That is an observation from clinicians, not a measured effect.

A lump that feels new

The same review advises breast clinics to keep the standard workup, examination plus imaging plus a biopsy where needed, for any lump in a woman on tirzepatide, and to recognize that new lumpiness may be benign tissue that weight loss has unmasked.[11] That is guidance for the clinic, not a reason to wait. Only an examination and imaging can tell an old benign lump from a new problem.

⚠ Weight loss can make an existing lump easier to feel. It does not make a lump safe to ignore. Any lump you have not noticed before should be examined, whether or not you are losing weight.

What weight loss does to a mammogram

Breast density, how much of the breast is gland rather than fat, matters for two reasons. Dense tissue can hide a tumor on a mammogram, and higher density is itself linked to breast cancer risk. Weight loss changes the ratio, and the result depends on how density is measured.

  • Measured as a percentage of volume, density usually rises. Fat leaves faster than gland, so the gland makes up more of what remains. In the 80-woman bariatric study, volumetric density went from 5.15% to 7.87%.[6] In a 12-month diet study of 46 women at higher risk, the same software showed density rising as weight fell, driven by lost fat.[10]
  • Read by a radiologist on the standard four-grade scale, it mostly does not move. The 180-woman study found no change in radiologist grades.[7] A 110-woman study at another center found grades went down.[9] A meta-analysis of seven bariatric studies (535 people) found no significant change in radiologist grades but a rise in software-measured density.[8]
  • An AI density score was unaffected. In the 46-woman diet study, a deep-learning measure trained on expert readings did not change significantly with weight.[10]

None of these studies involved a GLP‑1, and none tested whether cancers were found more or less often after weight loss. The 2026 tirzepatide review concluded that the available data do not suggest mammograms or other breast imaging become less accurate after weight loss.[11] Telling the imaging center about a large recent weight change gives the radiologist context when comparing with older images. Nothing in these studies suggests changing when you are screened.

Breast cancer risk, briefly

The full analysis lives in two other articles: the randomized answer and what the cohorts show. For breast cancer specifically, the evidence lines up like this.

Breast cancer and GLP-1 drugs: what each kind of evidence shows.
SourceDesignBreast cancer finding
Ko 2026, Annals of Internal MedicineMeta-analysis, 48 randomized trials, 94,245 peopleOdds ratio 0.95 (0.60–1.49): from 10 fewer to 12 more cases per 10,000; probably little or no effect, moderate certainty
Piccoli 2021, JCEMMeta-analysis, 52 randomized trials130 cancers in 48,267 treated vs 107 in 40,755 controls; relative risk 0.98 (0.76–1.26). Benign breast growths also no different
Saxenda labelLiraglutide 3 mg weight-loss trials17 of 2,379 women (0.7%) vs 3 of 1,300 on placebo (0.2%); label says too few to tell if related
McDonald 2026, JCO Oncology PracticeRetrospective records, women 45–80 having breast imagingLower odds of a breast cancer diagnosis in GLP-1 users (matched odds ratio 0.695); observational

The randomized trials are the anchor. Pooled together, they place breast cancer risk on a GLP‑1 near no effect, and the larger review held that result across semaglutide and tirzepatide trials.[13] The weakness is time: the trials were built to study weight or heart outcomes and ran a year or two, while breast cancer develops over many years.[13] An earlier pooling of 52 trials, prompted partly by the liraglutide imbalance, found no difference either.[14]

There were too few cases to determine whether these cases were related to SAXENDA. In addition, there are insufficient data to determine whether SAXENDA has an effect on pre-existing breast neoplasia.
— Saxenda (liraglutide) prescribing information, Adverse Reactions

That note is why “breast cancer” turns up next to GLP‑1 drugs online. Most of the cancers in those trials were estrogen- and progesterone-receptor positive.[5] No similar statement appears on the Ozempic or Wegovy labels,[1][2] or on Mounjaro’s or Zepbound’s.[3][4]

On the hopeful side, a 2026 records study of more than 100,000 women having breast imaging found fewer breast cancer diagnoses among GLP‑1 users after matching on age, BMI, density and diabetes.[15] Its own authors call for prospective trials. Observational numbers of this kind have misled before, as the cohorts article shows with a breast cancer study whose benefit vanished against a fairer comparison group. Weight loss itself has better support: across 10 cohorts and 180,885 women aged 50 and over, those who lost weight and kept it off had lower breast cancer risk than those whose weight stayed stable, with a hazard ratio of 0.68 for 9 kg or more, among women not using menopausal hormones.[18] No study has yet shown that weight lost on a GLP‑1 carries the same benefit. People already being treated for breast cancer face different questions, covered in GLP-1s in people being treated for cancer.

When to have a breast change checked

The CDC lists these as possible signs of breast cancer and says to see a doctor right away if any appear:[19]

  • A new lump in the breast or armpit, or a thickened or swollen area.
  • Nipple discharge other than breast milk, especially if bloody.
  • Irritation or dimpling of the skin of the breast, or redness or flaky skin around the nipple or on the breast.
  • A nipple that pulls inward, or pain in the nipple area.
  • Any change in the size or shape of the breast, or pain in any area of the breast.

Size change is on that list, which is awkward when weight loss is shrinking both breasts. Gradual, even change on both sides as weight falls is what the bariatric studies describe; a change in one breast only, or one that does not follow your weight, is the kind to have examined. That distinction is our reading, not a rule from the CDC. Men are included. Male breast enlargement appears in the adverse-event reports for both drugs, and a lump under the nipple in a man needs the same examination. The National Cancer Institute’s advice covers everyone: contact a doctor about any change in the breasts.[17] Mention the GLP‑1 and how much weight you have lost, so the person examining you can weigh it.

Frequently Asked Questions

It can, through weight loss rather than any direct effect of the drug. Breasts contain fat, and that fat shrinks as body fat does. No study has measured breast size on Ozempic or other GLP-1 drugs; after bariatric surgery, average breast volume measured on mammograms fell by about 39%. Drooping and some unevenness between the two sides can follow.
Breast pain is not listed as a side effect on the Ozempic, Wegovy, Mounjaro or Zepbound labels, and no published study links GLP-1 drugs to it. The FDA's adverse-event database holds 40 reports of breast pain for semaglutide out of about 73,000 reports, which are reports rather than proven side effects. Breast pain is common for other reasons, including normal cycle-related changes.
Neither the Mounjaro nor the Zepbound label lists it. The FDA's adverse-event database contains 14 reports of breast tenderness for tirzepatide among about 160,000 reports, which cannot show cause or a rate. As breast fat shrinks, a lump that was already there may become noticeable for the first time, so new tenderness or a lump should be checked.
Randomized trials do not show it. A 2026 meta-analysis of 48 trials put the effect of GLP-1 drugs on breast cancer at somewhere between 10 fewer and 12 more cases per 10,000 people, which it rated as probably little or no effect. The trials were short, so long-term risk is not settled. Saxenda's label notes more breast cancers on the drug than placebo in its trials but says the cases were too few to tie to the drug.
It may change the density reading. Fat leaves the breast faster than glandular tissue, so software-measured density often rises, while radiologists' grades usually stay the same. Studies after weight loss have not shown mammograms becoming less accurate. Nothing in them suggests changing when you are screened; telling the imaging center about a large weight change gives the radiologist context.
