Ozempic Vulva: What the Evidence Shows About Fat Loss and the Labia

No GLP-1 trial or label has measured the vulva. The mons pubis and labia majora are mostly fat, and these drugs remove fat everywhere. What plastic surgeons see after large weight loss, and what fillers, fat grafting and surgery can and cannot do.

Part of Body changes and sexual health on a GLP-1

The nickname describes a vulva that looks flatter, looser or older after rapid weight loss: less fullness in the mons pubis, the cushion of fat above the pubic bone, and in the outer lips, called the labia majora, with skin that no longer sits tight. No GLP-1 trial has measured the vulva, and no FDA label for semaglutide or tirzepatide mentions it. What is known is indirect. These drugs remove fat from the whole body, about three quarters of all weight lost.[4] The mons and labia majora are made largely of fat.[6] Plastic surgeons have described changes in this area after massive weight loss for years, long before these drugs.[7] The change is real anatomy. It is a weight-loss effect, not a known drug effect on the genitals.

About this article. Findings come from the published trials, the FDA labels on DailyMed, and peer-reviewed plastic surgery and gynecology papers, all listed in the references. Where a point is reasoning from those sources rather than a measured result, the text says so. Industry funding of treatment studies is noted where it exists. This page covers shape and volume. Itching, dryness, odor and infections are covered in vaginal dryness, odor and infections on a GLP-1. This is not medical advice.

What the term refers to

The vulva is the external genital area. Two parts of it are mostly fat under skin: the mons pubis and the labia majora. The inner lips (labia minora), the clitoris and the vaginal opening contain little fat. So when people describe deflation after weight loss, they are describing the two fatty structures, and sometimes the inner lips becoming more visible because the outer lips no longer cover them.

A 2023 anatomy study measured these structures in 214 women and called them the “fatty parts of the vulva.” It split the women into two body-mass-index groups. Estimated mons volume came to roughly 200 versus 300 mL between the groups, and a single outer lip to roughly 30 versus 40 mL. The authors concluded that BMI has to be considered in any procedure on the area.[6] Volume here tracks body size. That is the core of the issue.

Two different complaints travel under the same name. One is loss of volume: a flatter mons and thinner, wrinkled outer lips. The other is descent: a mons and labia that sag because the skin stretched while the person was heavier and did not shrink back. Many people have some of both. They are treated differently, which is why the distinction matters.

What the trials and labels show

Nothing about the vulva. PubMed indexes no study or case report on vulvar or labial appearance in people taking a GLP-1 drug. None of the four US labels (Ozempic, Wegovy, Mounjaro, Zepbound) uses the word vulva or labia anywhere. What the labels do state is where the weight comes from:

Semaglutide lowers body weight with greater fat mass loss than lean mass loss.[1]
— Wegovy prescribing information, section 12.2

The Zepbound label carries the same sentence for tirzepatide.[2] The numbers behind it come from scans. STEP 1 reported a mean weight drop of 14.9% at week 68 with semaglutide 2.4 mg.[3] In a SURMOUNT-1 substudy of 160 people, 73% of them women, whole-body DXA scans showed tirzepatide cut total weight by 21.3% and fat mass by 33.9% over 72 weeks. Fat made up roughly three quarters of what came off, an identical split to placebo.[4] In SURPASS-3, MRI showed tirzepatide reduced both abdominal subcutaneous fat and visceral fat, compared with insulin.[5]

None of these scans isolated the pubic area. DXA reports the trunk, arms and legs; the MRI substudy looked at the abdomen. So the following is inference, not measurement. If a third of body fat disappears and the mons and labia majora are mostly fat, they are very likely to shrink too, on roughly the same timeline as fat elsewhere. That is the same reasoning behind facial hollowing, covered in our skin changes Q&A.

What happens after massive weight loss, from the surgical literature

The best guide is the literature on people who lost large amounts of weight after bariatric surgery. It points to a result that differs from the online description. After massive weight loss, surgeons mostly see a mons that bulges and droops, not one that has disappeared. A 2011 series of 132 patients stated that both obesity and massive weight loss cause bulging and sagging of the mons, and graded it by how much fat remained and how far it had dropped.[9] A French series of 23 women described ptosis or a pubic bulge as a frequent result of massive weight loss.[12]

A California plastic surgeon who has written on the area since 2009 describes large, protruding mons and labia majora caused by fat deposits combined with sagging skin, and treats the outer lips by removing fat and skin, not by adding volume.[7] His later review put the point bluntly:

Even with weight loss, most of these deformities persist.[8]
— Alter, Plastic and Reconstructive Surgery, 2012

In practice this means three possible results after a large loss. Some people keep a fatty, drooping mons. Some get loose, emptied skin. Many get loose skin over leftover fat. Which one depends on starting size, how much was lost, age and skin quality. No study has sorted GLP-1 users into these groups. Volume loss of the outer lips is also common with age and menopause, independent of any drug, so the timing of a change does not prove its cause. Our page on GLP-1 drugs after menopause covers that group.

These changes are not only cosmetic. Among 31 women surveyed after mons lifts combined with abdominal surgery, average BMI had fallen from 52 to 31. Before surgery only 25.8% could see their own genitals; afterward all of them could. Self-rated appearance rose from 3.2 to 8.6 out of 10, hygiene improved for 61.3% and sex life for 51.6%.[10] A prospective comparison of 20 operated and 20 non-operated women found urinary symptoms improved at three months, but sexual function scores did not change.[11] All of these are small series from surgeons reporting their own results, and they involve far larger losses than most GLP-1 users have.

Even among clinicians who treat GLP-1 patients for cosmetic concerns, the vulva does not come up in the published data. A 2026 survey of 406 dermatologists, plastic surgeons and other providers listed midface volume loss, loose face and neck skin, sagging body skin and stubborn fat as the main concerns.[13] One of its authors works for Allergan Aesthetics, a filler maker.

Treatments in the literature, and their limits

Every option below was studied in people who were not on a GLP-1. Most outer-lip volume studies enrolled women whose change came with age or menopause. Results in a body that is still losing weight are unknown.

Procedures described in peer-reviewed studies of the mons and labia majora
ProcedureWhat it doesEvidenceKnown limits
Monsplasty (mons lift)Removes excess skin and fat and lifts the mons, usually during a tummy tuckCase series of 23 to 132 patients[9][12]; one small prospective comparison[11]Surgery, scars; no randomized trials
Labia majora reductionRemoves extra skin and fat from sagging outer lipsSurgeon case reports and reviews[7][8]Corrects excess, not deflation
Fat graftingMoves your own fat by injection into the outer lips4 studies, 183 patients in a 2017 review[14]Some fat is reabsorbed; untested in GLP-1 users
Hyaluronic acid fillerInjected gel adds volumeTwo small randomized trials[15][17] and an uncontrolled study[16]Temporary; not an FDA-approved site; rare serious complications
Calcium hydroxylapatite fillerInjected filler that also stimulates collagenOne randomized trial of 10 women[15]Too small to compare with hyaluronic acid

Fillers

A 2017 systematic review found nine studies on enlarging the outer lips. Only two were prospective, and the review graded the evidence at level 4, close to the bottom of the scale. It reported no major complications and high satisfaction, and called for randomized trials.[14] Since then, three studies stand out. A Brazilian trial randomized 10 women with sagging or deflated outer lips to hyaluronic acid or calcium hydroxylapatite; blinded raters judged 80% and 50% of results excellent, and the authors said the trial could not show either was better.[15] A French study of 72 women injected with one hyaluronic acid product reported that 97% rated themselves improved at 12 weeks and more than 92% through a year. It had no control group, and every author had a contract with the product’s maker.[16] An Italian trial randomized 76 women aged 40 to 65 to hyaluronic acid or no treatment and found better appearance and sexual function scores at six months, with no serious adverse events. Its authors called the results preliminary; two of them consult for a filler company.[17]

Two cautions apply. First, FDA approves dermal fillers to add volume to the lips, cheeks, chin and back of the hand, and recommends against injectable fillers for body contouring and enhancement.[21] The labia are not on the approved list, so any filler there is off-label. Second, serious complications are rare but documented. A Korean case report describes a woman who developed necrotizing fasciitis and sepsis a year after an outer-lip injection of unknown filler, and needed a muscle flap to close the wound.[20]

Fat grafting

Fat grafting uses your own fat, so there is no foreign material. Part of the graft does not survive. In a 2026 Egyptian series of 26 women given outer-lip fat grafts during tummy tucks, two needed revision after more than half the grafted fat was reabsorbed, and the author warned that combining the procedures can cause serious complications.[18] For GLP-1 users there is an added unknown. A 2026 scoping review found that no clinical or animal study has tested fat graft survival in people on these drugs. It raised a theoretical concern that the drugs’ effects on fat cells could work against graft survival, and labeled its own advice hypothesis-generating rather than evidence-based.[19]

Surgery and timing

Surgery is the only option shown to correct sagging skin and leftover bulk, and nearly all the data come from people whose weight had stabilized after bariatric surgery. Operating while weight is still falling risks a result that no longer fits a few months later. The general rule on waiting for a stable weight, and what stopping the drug afterward does, is set out in when it is safe to have the skin removed. Most GLP-1 users lose far less than bariatric patients and may never need surgery.

The American College of Obstetricians and Gynecologists says women considering cosmetic genital procedures, including outer-lip augmentation, should be told how little high-quality evidence exists. It lists risks including pain, scarring, altered sensation, painful sex and repeat surgery, and advises screening for body dysmorphic disorder.[22] That is a reasonable checklist before paying for any of the above.

What you can control

  • Rate of loss. No study has tested whether slower loss changes the vulva. Reviews of skin after weight loss point to speed and total amount lost as the main drivers of laxity (see our skin changes Q&A), so a slower dose escalation is a reasonable choice if this matters to you. It is a trade: slower loss means less loss in the same time.
  • Muscle. Resistance training and adequate protein keep more of the lean tissue that makes up about a quarter of the weight lost.[4] That helps the arms, thighs and buttocks. The mons and outer lips are mostly fat, so training is unlikely to fill them. This is inference from anatomy, not a measured result. More in GLP-1 hair loss and muscle loss.
  • Wait before procedures. Fillers and surgery done mid-loss aim at a moving target.
  • Separate symptoms from shape. Itching, burning, pain, sores, white or thickened patches, or new discharge are not volume loss. They need an exam. Dryness and infections on a GLP-1 are covered on our companion page.

Stopping the drug will not reverse skin that has already stretched. Regained weight adds fat back, including here, along with the health risks the drug was treating. If the change bothers you enough to consider stopping, raise it with the prescriber first; slowing or holding the dose is usually the first step.

Frequently Asked Questions

It is an informal name, not a diagnosis, for changes to the external genitals after rapid weight loss on a GLP-1 drug: less fullness in the mons pubis and labia majora, and looser skin. Both structures are mostly fat, so they lose volume when body fat falls.
The change is real, but it is not specific to Ozempic. No GLP-1 trial or label has measured the vulva. Weight loss on these drugs is about three quarters fat, and plastic surgeons have long described changes to the mons and labia majora after large weight loss from bariatric surgery.
Not through any known direct effect. The labia majora are mostly fat, and semaglutide and tirzepatide reduce body fat, so the outer lips may lose volume along with the rest of the body. Nobody has measured how often or how much in GLP-1 users.
Itching is not caused by fat loss. Dryness, irritation, yeast infections and skin conditions are separate issues; our companion page on vaginal dryness, odor and infections on a GLP-1 covers them. Persistent itching, sores or white patches need an exam by a clinician.
Small studies in women with age-related volume loss report good satisfaction with hyaluronic acid filler and fat grafting, but none studied GLP-1 users. Fillers are temporary and the labia are not an FDA-approved filler site. Grafted fat is partly reabsorbed. Waiting until weight is stable is sensible.
Regaining weight adds fat back, including in this area, but stretched skin does not fully tighten again, and the health problems the drug treated return too. Slowing dose increases or holding a dose is a gentler option to discuss with your prescriber.
