Rybelsus for Weight Loss: How Much Weight People Lose, and Why It's Less

Rybelsus is approved for type 2 diabetes, not weight. In its trials the 14 mg dose took off 3.1 to 4.4 kg in 26 weeks. Why the Wegovy pill does far more, what is known in people without diabetes, and why insurers won't cover it for weight.

Part of The next obesity drugs, and what their trials showed

Rybelsus does cause weight loss, but not much, and it is not approved for it. In the trials behind its FDA label, adults with type 2 diabetes on the top 14 mg dose lost 3.1 to 4.4 kg (about 7 to 10 pounds) over 26 weeks, and those on 7 mg lost 2.2 to 2.4 kg.[1] The 3 mg starter dose did almost nothing for weight.[2] The FDA approved Rybelsus only for blood sugar control and heart risk reduction in type 2 diabetes.[1] For weight loss, the same molecule is sold as the Wegovy tablet at a 25 mg dose, which took off 13.6% of body weight in its own trial.[7]

About this article. Built from the current FDA label for Rybelsus (which since 2026 also covers Ozempic tablets), the published PIONEER, SOUL and OASIS trials, the Wegovy label, and the written coverage rules of two public and private plans. It explains what the evidence shows. Whether a GLP-1 suits you, and which one, is a decision for you and a prescriber who knows your history.

What Rybelsus is approved for

Rybelsus is semaglutide, the same drug as in Ozempic and Wegovy, in a tablet taken once a day. Its label lists two uses, both in adults with type 2 diabetes: improving blood sugar control alongside diet and exercise, and making a heart attack, a stroke or a cardiovascular death less likely in people at high risk of those events.[1] Weight loss is not among them, and the label says it has not been shown to be safe and effective in children.[1]

The heart indication comes from SOUL, a trial of 9,650 people with type 2 diabetes and heart disease, kidney disease or both, followed for a median of just over four years. Rybelsus at up to 14 mg cut major cardiovascular events by 14% against placebo (hazard ratio 0.86, 95% CI 0.77 to 0.96).[5][1] That result belongs to people with diabetes. It says nothing about using the tablet for weight in someone without it.

The dose schedule is short. Everyone starts on 3 mg for 30 days, a step the label calls “not effective for glycemic control” and which exists to let the stomach adjust. Then 7 mg, and from day 61, 14 mg if blood sugar needs more help.[1] Each tablet is taken with the stomach empty and 4 ounces of plain water at most, and then at least 30 minutes pass without food, drink or other pills.[1] For the full dosing table and side-effect list, see the Rybelsus drug page.

How much weight people lost on Rybelsus in the PIONEER trials

Body weight was a key secondary outcome in the PIONEER program, so every main trial measured it. The label reports each one at 26 weeks, counting everyone randomized whether or not they kept taking the drug.[1] All of the people in these trials had type 2 diabetes.

Average weight change at 26 weeks in the PIONEER trials, as printed in the Rybelsus label, Section 14. All participants had type 2 diabetes.

In PIONEER 1, people on 14 mg started at 88.1 kg on average, so the 3.7 kg they lost was about 4% of their weight. Against placebo, the extra loss was 2.3 kg at 14 mg, 0.9 kg at 7 mg and 0.1 kg at 3 mg.[2] Only the 14 mg result was clearly better than placebo for weight. When the analysis counted only people who stayed on the tablet without rescue drugs, the 14 mg advantage rose to 2.6 kg.[2]

The active-comparator trials tell the same story from another angle. In PIONEER 3, 7 mg and 14 mg took off 1.6 kg and 2.5 kg more than sitagliptin, while 3 mg failed to show it was even as good as sitagliptin on blood sugar.[3] In PIONEER 4, 14 mg beat the injected diabetes drug liraglutide on weight (−4.4 kg against −3.1 kg).[4] Against empagliflozin, an SGLT2 pill that also lowers weight, the result was a tie.[1]

On the highest Rybelsus dose, adults with type 2 diabetes lost 3.1 to 4.4 kg in 26 weeks. On the Wegovy tablet, adults without diabetes lost 13.6% of their weight in 64 weeks.

Why Rybelsus causes less weight loss than the Wegovy pill

The Wegovy tablet is the same molecule taken the same way. In OASIS 4, 307 adults without diabetes and with obesity, or overweight plus a related condition, took 25 mg or placebo daily. At 64 weeks the tablet group had lost 13.6% of body weight, against 2.4% on placebo in the FDA label’s analysis (2.2% in the published paper).[7][8] Three things separate that result from the PIONEER figures.

The dose

Very little oral semaglutide reaches the blood. The label puts absorption from Rybelsus, at any of its 3, 7 and 14 mg strengths, at 0.4% to 1%, against 1% to 2% for the newer Ozempic tablets, and the Wegovy label gives about 1% to 2% for its tablets.[1][7] Combine a 25 mg dose with that better absorption and the Wegovy tablet delivers far more drug than Rybelsus 14 mg. A head-to-head in diabetes shows the dose effect directly: in PIONEER PLUS, 25 mg and 50 mg beat 14 mg on both blood sugar and body weight over 52 weeks.[6] An earlier obesity trial, OASIS 1, gave 50 mg a day to adults without diabetes and recorded a 15.1% loss at 68 weeks, against 2.4% on placebo.[9]

Who was in the trials

People with type 2 diabetes lose less weight on GLP-1 drugs than people without it, even at the same dose. The injected Wegovy dose of 2.4 mg shows this cleanly. It took off 14.9% at 68 weeks in adults without diabetes (STEP 1) and 9.6% in adults with it (STEP 2), against 3.4% on placebo in STEP 2.[7][10] Every PIONEER trial enrolled people with diabetes, so their figures are pulled down for that reason alone.

How long the trials ran

The PIONEER weight figures in the label are measured at 26 weeks; the OASIS figures at 64 and 68 weeks. A six-month reading catches people partway. PIONEER 3 followed weight to 78 weeks, and 14 mg still produced significantly more weight loss than sitagliptin at that point.[3] Even so, nothing in the PIONEER results comes near the OASIS figures. For a fuller comparison of the oral and injected forms, see whether the Wegovy pill is as good as the shot and the Wegovy vs Rybelsus comparison.

Rybelsus for weight loss in people without diabetes

No trial in the Rybelsus label enrolled people without diabetes, so there is no label figure for what 7 or 14 mg does in that group.[1] The weight-loss trials of oral semaglutide in people without diabetes, OASIS 1 and OASIS 4, used 50 mg and 25 mg, doses Rybelsus does not come in.[9][8] Anyone quoting a Rybelsus weight figure for a non-diabetic patient is either borrowing those higher-dose results or extrapolating from the diabetes trials.

A doctor can legally prescribe Rybelsus off-label for weight. Few have a reason to. Since the FDA approved the Wegovy tablet for adults who have obesity, or who are overweight with one or more weight-related conditions, an oral semaglutide approved for exactly this use exists at a dose shown to work.[7] Lilly’s orforglipron, sold as Foundayo, is a second approved weight-loss pill without the fasting rule.

The side effects, meanwhile, come at full strength. In the label’s placebo-controlled pool, nausea hit 11% of people on 7 mg and 20% on 14 mg, against 6% on placebo, with abdominal pain, diarrhea, decreased appetite, vomiting and constipation following.[1] The tablet carries the same boxed warning about thyroid C-cell tumors in rodents as the injections.[1]

Why insurance won’t cover Rybelsus for weight loss

Plans that cover Rybelsus usually tie it to a type 2 diabetes diagnosis, and many check that diagnosis through prior authorization or a diagnosis code on the claim. Two written policies show how this looks in practice:

  • Blue Cross Blue Shield of Michigan’s Medicare Advantage plans require a type 2 diabetes diagnosis before approving Rybelsus, Ozempic and other diabetes GLP-1s for new starts, effective May 10, 2024. Its table states plainly that these drugs are not covered if used for weight loss.[11]
  • California’s Medicaid pharmacy program, Medi-Cal Rx, keeps a type 2 diabetes diagnosis restriction on Rybelsus and Ozempic, and from January 1, 2026 stopped covering the GLP-1s on its list for weight loss, apart from medical-necessity review for members under 21.[12]
  • Medicare’s GLP-1 Bridge, the temporary program that pays for weight-loss GLP-1s, names Wegovy, Zepbound and Foundayo, not Rybelsus. People with type 2 diabetes are excluded from the Bridge because their regular Part D plan may cover a GLP-1 for diabetes instead.[13]

Without a diabetes diagnosis, then, a Rybelsus prescription is likely to be paid out of pocket, at a dose that delivers a fraction of the weight loss of the drugs that are covered for it. If you are trying to get a weight-loss GLP-1 approved, our guides to prior authorization and appeals and the Medicare GLP-1 Bridge walk through the rules. Self-pay prices for the oral alternative are in what the Wegovy pill costs.

Rybelsus and the new Ozempic tablets

The Rybelsus label on DailyMed is now shared with a second product, Ozempic tablets, in strengths of 1.5, 4 and 9 mg; the cartons call them a new formulation of semaglutide.[1] The uses, the fasting rule and the warnings are the same. The doses are not interchangeable milligram for milligram: the label maps Rybelsus 7 mg to Ozempic tablets 4 mg and Rybelsus 14 mg to 9 mg, and gives the new tablets a higher absorption, 1% to 2% against 0.4% to 1%.[1] The label found no clinically significant difference in blood levels between the matched doses, so the weight figures above should carry over to the matching Ozempic tablet dose, although no trial measured weight on the new tablets directly. Neither product is approved for weight loss.[1] What the change means for anyone already on Rybelsus is covered in whether Rybelsus is being discontinued, and the injected and oral Ozempic products are compared in Rybelsus vs Ozempic.

Rybelsus has also been tested directly against the newest weight-loss pill. In a 52-week diabetes trial, orforglipron lowered blood sugar more than the 7 and 14 mg doses of oral semaglutide, at the cost of more people stopping treatment; the details are in the first pill-versus-pill trial.

Frequently Asked Questions

Yes, modestly. In the trials behind its label, adults with type 2 diabetes on 14 mg lost 3.1 to 4.4 kg (about 7 to 10 pounds) over 26 weeks, and those on 7 mg lost 2.2 to 2.4 kg. In PIONEER 1 the 14 mg group lost 2.3 kg more than placebo; the 3 mg starter dose made almost no difference to weight.
No. Its label covers two uses in adults with type 2 diabetes, controlling blood sugar and, for those at high risk, making a heart attack, a stroke or a cardiovascular death less likely. The oral semaglutide approved for weight loss is the Wegovy tablet at 25 mg.
There is no label figure, because none of the Rybelsus trials enrolled people without diabetes. The oral semaglutide trials in people without diabetes used higher doses: 25 mg gave a 13.6% loss at 64 weeks in OASIS 4, and 50 mg gave 15.1% at 68 weeks in OASIS 1. Rybelsus tops out at 14 mg.
Mostly dose. Rybelsus 14 mg is absorbed at 0.4% to 1%, while the Wegovy tablet is 25 mg absorbed at about 1% to 2%, so far more drug reaches the blood. The PIONEER trials also enrolled only people with type 2 diabetes, who lose less weight on GLP-1 drugs, and measured weight at 26 weeks rather than 64 or more.
Rarely. Plans that cover Rybelsus generally require a type 2 diabetes diagnosis, often checked through prior authorization, and some, such as Blue Cross Blue Shield of Michigan’s Medicare Advantage plans, state outright that it is not covered for weight loss. Medicare's GLP-1 Bridge for weight loss covers Wegovy, Zepbound and Foundayo, not Rybelsus.
They share one FDA label, the same uses and the same instructions, but the doses differ because Ozempic tablets are absorbed better. The label maps Rybelsus 7 mg to Ozempic tablets 4 mg and Rybelsus 14 mg to 9 mg, and says the two are not substitutable milligram for milligram.

Is Rybelsus Being Discontinued? What Happened and the Ozempic Pill Switch

Yes, in the US. The FDA posted Rybelsus 3, 7 and 14 mg as discontinued on June 4, 2026, replaced by reformulated Ozempic tablets 1.5, 4 and 9 mg. Same semaglutide, same label, no safety recall. The label's conversion: 7 mg to 4 mg, 14 mg to 9 mg.
