FUPA and GLP-1 Weight Loss: What Shrinks It and What Does Not
No trial has measured the fat over the pubic bone on Ozempic, Wegovy, Mounjaro or Zepbound. What the scans show about belly fat, why ab exercises do not target it, why loose skin stays, and when insurers cover panniculectomy.
You cannot exercise the FUPA away on its own, and no drug targets it. It shrinks, if it shrinks, as part of overall fat loss, and that is how a GLP-1 drug affects it too. No trial of semaglutide or tirzepatide has measured the fat over the pubic bone; the scans show large losses of total, belly and visceral fat, and the pubic pad is very likely to follow.[9][11] What weight loss cannot remove is stretched skin. After a large loss, a drooping pubic area or a hanging lower-belly apron often remains, and only surgery takes it away.[13][2]
What the FUPA actually is
The slang lumps together two separate problems, and the fix depends on which one you have.
The first is the mons pubis, the cushion of fat that sits on the pubic bone in both sexes. In a 2023 anatomy study of 214 women, its estimated volume was about 200 mL in one body-mass-index group and 300 mL in the other, so it varies a great deal with body size.[1] Plastic surgeons grade problems here by two things: how much fat there is, and how far the area has dropped.[2]
The second is the panniculus, or pannus: an apron of skin and fat from the lower abdomen that drapes over the pubic area and, in bigger cases, the upper thighs. Surgeons grade it from 1 to 5 by how far it hangs. In one center’s bariatric patients, removed aprons weighed from 5 to 54.5 kg, and people with grades 3 to 5 had more back pain and more hygiene problems.[3] A large panniculus is not only a cosmetic issue. Heat and moisture get trapped beneath the apron, and that breeds yeast rashes (candidal intertrigo), dermatitis and, in severe cases, swelling and inflammation of the fat itself.[4]
Many people have both: extra fat on the mons and a lower-belly fold above it. Below a certain size, both respond to fat loss. Once the skin has stretched and lost its spring, the fat can go while the overhang stays.
The short answer, method by method
| Method | Best evidence | What it shows |
|---|---|---|
| Ab exercises alone | Small randomized and controlled trials[14][15][16] | Mostly no local fat loss; two small recent trials report a modest one[17][18] |
| Diet and overall weight loss | Body composition trials[9][10] | Removes fat everywhere, including the belly; no study measured the pubic pad |
| Semaglutide or tirzepatide | FDA labels; DXA and MRI substudies[5][6][9][11] | About three quarters of weight lost is fat; belly and visceral fat fall; pubic pad not measured |
| Fat freezing (cryolipolysis) | Meta-analysis of 30 studies[19] | A few centimeters off the waist; uncommon fat overgrowth[20] |
| Monsplasty (pubic lift) | Case series[2][21] | Lifts and reduces the mons; usually done with a tummy tuck |
| Panniculectomy | Surgical series[3][22] | Removes the hanging apron; higher complication rates than a tummy tuck |
What GLP-1 trials measured, and what they did not
Both drug labels say the same thing about where the weight comes from. For both semaglutide and tirzepatide, the label reports that more of the lost weight was fat than lean mass.[5][6] Neither label says anything about the lower abdomen or the pubic area.
The size of the loss is well established. STEP 1 ran for 68 weeks; mean weight fell 14.9% in the semaglutide 2.4 mg arm and 2.4% in the placebo arm.[7] SURMOUNT-1 ran for 72 weeks; the tirzepatide 15 mg arm came down 20.9%, placebo 3.1%.[8] A SURMOUNT-1 substudy scanned 160 participants, most of them women, with DXA, a low-dose X-ray scan that sorts the body into fat, bone and lean tissue. On tirzepatide, total fat fell by 33.9%, and fat made up roughly three quarters of the weight that came off, a split no different from placebo.[9] A 2026 review of 35 randomized trials found that weight loss on these drugs consistently came with less total fat and less visceral fat, the deep fat that surrounds the organs.[10]
MRI gives a closer look at the belly. In the SURPASS-3 MRI substudy of 296 adults with type 2 diabetes, tirzepatide reduced both visceral fat and abdominal subcutaneous fat (the pinchable layer under the skin) more than insulin did.[11] A later analysis compared the same participants with people of matching sex and BMI from a large UK imaging study. After treatment, visceral and liver fat were lower than expected for their new size, while abdominal subcutaneous fat was slightly higher than expected. The authors read this as a shift toward a healthier pattern, with the deep fat going first.[12] That analysis was exploratory, in people with diabetes, and written by authors including employees of the drug’s maker.
None of these scans isolated the pubic area. DXA reports the trunk, arms and legs as blocks; the MRI work looked at the abdomen. So the following is inference, not measurement. The mons and the lower-belly fold are mostly fat under the skin. When a third of body fat disappears, they shrink too. The MRI pattern suggests that the pinchable belly layer may lag behind the deep visceral fat, which fits the common experience that the lower belly is among the last places to change. It does not show that the drug spares it.
Roughly a quarter or more of the weight that comes off on these drugs is lean tissue. Eating enough protein and lifting weights during treatment help hold on to it. That is covered in GLP-1 hair loss and muscle loss and fat-to-muscle ratio: surgery versus drugs.
Why the overhang can stay after the fat goes
Years of stretching can leave skin that never fully retracts. Plastic surgeons who operate after large weight loss describe the result in the pubic area plainly: fat that remains, plus a fat pad and skin that have dropped. A California surgeon who treats both men and women wrote that weight loss by itself leaves most of these problems in place, and that fixing them means lifting and removing pubic skin and fat.[13] A 2011 series of 132 patients made the same point, that massive weight loss as well as obesity causes the mons to bulge and sag.[2]
Most of that literature comes from bariatric surgery patients, who usually lose more than GLP-1 users do. Smaller losses leave less excess skin, and many people on a GLP-1 will never have an overhang worth operating on. No study has sorted GLP-1 users by how much pubic or lower-belly skin they are left with. In men, the same overhang can cover the base of the penis; that is covered in our penis size review.
Spot reduction: what the exercise trials found
The idea that crunches burn the fat on top of the abs has been tested for decades. Most of the controlled trials found that they do not.
- Abdominal exercise, 6 weeks. 24 adults were randomized to seven ab exercises five days a week or to no exercise, on a steady diet. The exercise group could do more curl-ups, but there was no effect on belly fat by DXA, waist size or skinfolds above the hip.[14]
- One arm trained, 12 weeks. 104 adults trained only their non-dominant arm. Skinfold calipers suggested a little local fat loss in men, but MRI, the better measure, found fat loss spread across both arms equally in men and women.[15]
- One leg trained, 12 weeks. 11 adults did very high-repetition leg presses with one leg. They lost body fat overall, but the loss showed up in the arms and trunk, not in the trained leg.[16]
Two recent small trials disagree. In a Norwegian study, 16 overweight men either ran on a treadmill or combined running with heavy ab and torso exercise, with energy use matched. After 10 weeks, the ab group lost about 1.2 kg of trunk fat by DXA and the running-only group lost none from the trunk, even though total fat loss was similar.[17] An Italian study of 14 volunteers found that circuit training aimed at the belly reduced abdominal subcutaneous fat over 8 weeks.[18]
Taken together: if targeted exercise moves local fat at all, the effect is small, it has been shown only in tiny groups, and the positive trial measured the whole trunk rather than the lower belly. It cannot shrink a hanging fold of skin. Core exercise is still worth doing for strength and posture. It is just not a fat-removal tool for one spot.
Fat freezing and other non-surgical options
Cryolipolysis, sold as fat freezing, cools a pinched area of fat to kill some fat cells. A 2025 meta-analysis of 30 studies and 3,158 people found that, three months after treatment, waist circumference was about 3.6 cm smaller and fat thickness above the hip about 5 mm thinner than at baseline. Side effects were common but mostly minor: about half reported numbness and 44.5% redness. The lead author offers the treatment in their own clinics.[19] A separate 2025 review put the rate of paradoxical adipose hyperplasia, where the treated fat grows larger instead of smaller, at about 1 in 455 patients, higher than manufacturers report.[20]
These devices remove a modest amount of pinchable fat. They do not tighten loose skin, and they were not studied in people on GLP-1 drugs or in people whose weight is still falling.
Monsplasty and panniculectomy
Monsplasty, a pubic lift, removes excess skin and fat over the pubic bone and anchors what remains higher. It is usually done as part of a tummy tuck, since the lower-belly incision gives access.[2] In a Belgian study of 25 people with at least moderate sagging of the mons, followed for three months, body image and sexual function scores improved, and some reported better hygiene, an easier sex life and being able to see their genitals again. There were no major complications in those 25 or in 80 earlier patients reviewed from the same clinic.[21] These are small series from surgeons reporting their own work.
Panniculectomy removes the hanging apron of skin and fat below the belly button. It is a functional operation: the abdominal muscles are not tightened and the navel stays where it is, which is what separates it from a tummy tuck. Tightening the muscles means repairing a diastasis recti, the widened seam down the middle of the abdomen. It carries more risk than a tummy tuck because the patients are heavier and the tissue larger. At Johns Hopkins, 30-day complications occurred in 21.2% of panniculectomies against 9.7% of tummy tucks, though rates evened out by six months.[22] In a larger series of 563 patients, 34.3% had at least one complication. Higher BMI and smoking both raised the risk of wound problems.[4]
For people who reached their weight with a GLP-1, a 2026 meta-analysis of 12 observational studies linked the drugs to more delayed wound healing and more thickened scars after body contouring, and to fewer fluid collections (seromas) than in patients who had had bariatric surgery.[23] None of the studies was randomized. Whether to hold the drug around surgery is a decision for the surgeon and anesthesia team.
Timing also matters. Skin removed while weight is still falling can loosen again, and weight regained after stopping the drug changes the result. The published timing evidence is in when it is safe to have the skin removed, and what “stable” means in practice is in what a plateau actually means.
When insurance pays
Insurers treat these operations very differently. Panniculectomy can be covered when the apron causes a medical problem. Tummy tucks and pubic lifts done for appearance are generally not. Aetna’s published policy is a typical example of the rules. It considers panniculectomy medically necessary only when all of these are documented:[24]
- The panniculus hangs below the level of the pubic bone, shown in front and side photographs.
- It causes chronic intertrigo, a rash or infection in the skin fold, that keeps coming back over three months of prescribed treatment or does not respond to it, with photographs taken with the apron lifted.
- Over the latest three months, weight has held steady, typically staying within 5% of the plateau.
- After bariatric surgery, a full 12 months or more must pass from the point the weight leveled off.
The same policy lists tummy tucks, panniculectomy that does not meet those criteria, and panniculectomy for back pain as cosmetic.[24] A separate Aetna policy lists a pubic lift (mons pubis pexy) done to change the appearance of the female genitals as cosmetic.[25] Neither policy mentions GLP-1 drugs. For someone losing weight on one, the stability rule is the one that usually decides timing: coverage generally has to wait until the weight stops moving. Other insurers set their own criteria, so ask for the written policy before booking a consultation.
What is worth doing
- Lose fat overall. It is the only thing shown to shrink belly fat, and the pubic pad is likely to follow. Whether that is through diet, a GLP-1 drug or both is a question of how much you need to lose.
- Keep the muscle. Strength training and enough protein limit lean-tissue loss. They will not burn the pad directly.
- Treat the skin fold. A rash, smell or broken skin under a fold is a medical problem, not a cosmetic one. Keeping the fold dry and getting treatment also creates the record an insurer will ask for.
- Wait for a stable weight before any procedure. Devices and surgery done mid-loss treat a body that is still changing.
- Separate fat from skin. If you can pinch thick fat, more fat loss will help. If what remains is mostly loose, thin skin, only surgery removes it.
Many people carry fat in this area at every size, and it is not a sign of ill health in itself. The reasons to act are discomfort, skin problems, or that it bothers you, and those are all legitimate.
