How the titration schedule works

The prescribing information [1] fixes the sequence. Four weeks at 2.5 mg once a week come first, then 5 mg weekly. After that, a prescriber may go up in 2.5 mg steps, never sooner than 4 weeks on the current dose, according to how you respond. Nothing above 15 mg a week is recommended.

A KwikPen is prefilled and holds a single dose. One pen, one injection, and then it is thrown away. Nothing is dialed and no clicks are counted toward a dose: the whole fixed amount goes in at once.

Pen colors

Every strength is labeled in its own color under the approved labeling [1][2] — which matters most when several pens are in the same fridge and one of them is not this week’s:

2.5 mg: Yellow
5 mg: Reddish-brown (maroon)
7.5 mg: Green
10 mg: Dark blue
12.5 mg: Purple
15 mg: Blue-gray

Storage

A refrigerator, at 36°F to 46°F (2°C to 8°C), is where Mounjaro belongs [1]. It will hold at room temperature — anything up to 86°F / 30°C — for 21 days. Keep it out of the light, keep it out of the freezer, and if a pen has frozen at any point it cannot be used.

Administration

It goes subcutaneously, into the abdomen, the thigh or the upper arm, and the site changes with every injection. Skin that is sore, bruised, reddened or hardened is skin to leave alone [1][2].
