What each label actually says

Every one of these drugs sets its own rule for a missed dose, in its own label. What separates them is the window: how long after the day it was due a dose can still be taken at all.

  • Wegovy (semaglutide 2.4 mg) [1]. Inside 5 days of the scheduled day, take it as soon as you can. Past 5 days, let it go and take the next one when it falls due. Two or more missed in a row is a different situation: going back to a lower dose can spare you the GI effects of restarting at full strength, and that is a call for your prescriber.
  • Ozempic (semaglutide 0.5/1/2 mg) [2]. The same 5 days. Inside it, take the dose; past it, skip and pick the schedule back up.
  • Zepbound (tirzepatide) [3]. A shorter window, 4 days — 96 hours. Inside it, take the dose as soon as you can; past it, skip to the next scheduled one. Whatever happens, leave at least 3 days (72 hours) between any two doses.
  • Mounjaro (tirzepatide) [4]. Identical to Zepbound: 4 days, 96 hours, and never less than 3 days between doses.
  • Foundayo (orforglipron) [5]. A daily tablet rather than a weekly injection, so there is no window to speak of. Miss one and it is simply missed; take the next at its usual time. Never two in a day.

Why the windows differ

It comes down to how long each molecule lasts. Semaglutide has a half-life near 7 days and tirzepatide near 5, which is exactly why Wegovy and Ozempic can allow 5 days to make a dose up while Zepbound and Mounjaro allow 4. The reason for any limit at all is the same in both cases: a late dose landing too near the next one puts two peaks on top of each other, and the side effects follow.

Orforglipron (Foundayo) is a daily oral GLP-1 with a much shorter half-life (~25-35 hours), so the guidance is simply to skip and take the next day’s dose at the regular time.
