Constipation is common enough to be listed in Section 6.1 of both labels: the Wegovy label reports 24% against 11% on placebo, and the Zepbound label reports it by dose — 17% at 5 mg, 14% at 10 mg and 11% at 15 mg, against 5%. Important tip

Add about 5 g a week, no faster, and drink more water alongside it. Arriving at a high-fiber diet in one step brings bloating and gas with it, which is the last thing worth adding to the GI symptoms a GLP-1 already causes.

How the calculator works

  • The National Academies figures (2005) [1]: the baseline, set by age and sex. Women 51 and over are given 21 g a day, women 19 to 50 are given 25 g, men 51 and over 30 g, and men 19 to 50 the highest at 38 g.
  • The USDA Dietary Guidelines (2020-2025) [2] put the same thing another way: 14 g for every 1,000 kcal you eat.
  • The constipation adjustment: tick both the GLP-1 box and the constipation box and 5 g a day is added on top, pushing you toward the upper end of your band. The AGA’s 2021 guidance treats that as the first thing to try [6].
  • Getting there slowly: about 5 g more each week is what the AGA advises [6], which is what keeps the bloating and gas down. Give the calculator what you eat now and it lays the weeks out for you.

Why GLP-1 patients need fiber

These drugs slow the stomach down and quieten the gut, and constipation follows directly from that [3][4]. Eating less compounds it: unless you are deliberately choosing high-fiber food, less food means less fiber. Slower transit, less bulk and often less fluid together are why constipation outlasts most of the other side effects.

Before anyone reaches for a laxative, the first move is fiber and enough water to go with it. The soluble kind — oats, beans, psyllium — softens and adds bulk; the insoluble kind, in bran and vegetables, gets things moving.

Important: increase gradually

Going from a low-fiber diet straight to the target brings bloating, gas and abdominal discomfort with it — the last things you need on top of the GI effects a GLP-1 is already producing. Roughly 5 g more per week is the AGA’s recommendation [6], and the schedule above is built to it.
