Mounjaro Heart Protection: What SURPASS-CVOT Found and What FDA Approved
In August 2026 FDA gave Mounjaro a heart-risk indication for high-risk adults with type 2 diabetes. SURPASS-CVOT found 12.2% vs 13.1% major heart events against Trulicity: non-inferior, not superior. Zepbound has no heart indication; SURMOUNT-MMO is due in 2027.
Yes. In August 2026, FDA approved Mounjaro (tirzepatide) for lowering the chance of a heart attack, a stroke or death from cardiovascular causes, in adults with type 2 diabetes whose risk of such events is high.[2][3] The approval rests on SURPASS-CVOT, a trial of 13,299 people with type 2 diabetes and established heart or artery disease, which compared Mounjaro with Trulicity (dulaglutide), a drug already shown to cut heart events. Mounjaro had 8% fewer events than Trulicity; that met the trial’s goal of proving it no worse, but it did not prove it better.[1] Zepbound, the same drug sold for weight loss, does not have a heart indication. Its heart trial in people with obesity is still running.[4][5]
What FDA approved, and for whom
Mounjaro’s label now lists two uses. The first is the original one: better blood-sugar control in type 2 diabetes, with diet and exercise, for adults and for children from age 10. The second, added in August 2026, reads:[2]
“to reduce the risk of major adverse cardiovascular (CV) events (CV death, non-fatal myocardial infarction (MI), or non-fatal stroke) in adults with type 2 diabetes mellitus who are at high risk for these events.”— MOUNJARO prescribing information, section 1
Only adults who have type 2 diabetes fall under it. It does not cover people who take tirzepatide for weight loss without diabetes, and it does not make Mounjaro a weight-loss drug. The approval was announced on August 28, 2026.[3] Trulicity, the drug it was tested against, already carried a heart indication of its own, as the Mounjaro label notes.[2]
SURPASS-CVOT: who was in the trial
To join, participants had to be at least 40, have type 2 diabetes with an HbA1c between 7.0% and 10.5%, a BMI of 25 or more, and established cardiovascular disease such as coronary artery disease, a prior heart attack or stroke, or peripheral artery disease.[2] After 134 people randomized in error were removed, 6,586 received tirzepatide (up to 15 mg weekly) and 6,579 received dulaglutide 1.5 mg weekly. Their average age was 64, 29% were women, the average BMI was 32.6 and they had had diabetes for about 15 years.[1] Most were already well treated for heart risk: 86% were on a statin.[2]
The comparison matters. Most heart trials of diabetes drugs test against placebo. SURPASS-CVOT tested tirzepatide against Trulicity, which had already reduced heart events in its own trial. Beating an active drug is a much higher bar than beating a dummy injection, and the trial was designed first to show that tirzepatide was no worse. The Mounjaro and Trulicity products are compared side by side at Trulicity vs Mounjaro.
The results
| Outcome | Tirzepatide | Dulaglutide | Hazard ratio (95% CI) | Status |
|---|---|---|---|---|
| CV death, heart attack or stroke (primary) | 801 (12.2%) | 862 (13.1%) | 0.92 (0.83 to 1.01)* | Non-inferior; superiority not shown |
| Cardiovascular death | 5.5% | 6.2% | 0.89 (0.77 to 1.02) | Not formally tested |
| Non-fatal heart attack | 4.4% | 4.7% | 0.93 (0.80 to 1.09) | Not formally tested |
| Non-fatal stroke | 3.2% | 3.3% | 0.97 (0.80 to 1.16) | Not formally tested |
| Death from any cause | 8.5% | 10.1% | 0.84 (0.75 to 0.94) | Not controlled for multiple testing |
*95.3% confidence interval. Primary-outcome counts from the trial paper; component and death rates from Table 10 of the Mounjaro label, which reports the all-randomized, treated population.[1][2]
The primary result was a hazard ratio of 0.92, meaning about 8% fewer heart attacks, strokes and cardiovascular deaths with tirzepatide. The upper end of the confidence interval was 1.01, inside the trial’s non-inferiority margin of 1.05 but just above 1.00, which is the line for superiority. So the trial proved tirzepatide was at least as good as dulaglutide (P = 0.003) and did not prove it was better (P = 0.09).[1]
Deaths from any cause were 16% lower with tirzepatide, 8.5% against 10.1%. The label adds a footnote that this result was not controlled for testing many outcomes, so it cannot be read as proof.[2] Side effects were similar overall, with more nausea, vomiting and diarrhea on tirzepatide.[1]
The 28% figure, and where it comes from
Some coverage reported that tirzepatide cut heart events by 28%. That number does not come from SURPASS-CVOT itself. It comes from a separate, pre-specified analysis that estimated what tirzepatide would have done against placebo by borrowing the placebo group from REWIND, Trulicity’s own outcome trial, and matching participants on their characteristics. That analysis gave a hazard ratio of 0.72 against an imputed placebo.[6] Its authors call it an indirect, exploratory comparison. It is a reasonable estimate, but no one was randomized to placebo, and the direct result remains 0.92 against Trulicity.
Kidneys: a clearer difference
In a pre-specified exploratory analysis of the same trial, a composite of serious kidney outcomes occurred in 6.0% on tirzepatide and 7.6% on dulaglutide, a 23% lower risk over a median of four years. The gap was driven mostly by fewer people developing heavy protein in the urine.[7] The details, including what “exploratory” means for how much weight to give it, are in the kidney analysis.
Does Zepbound protect the heart?
Zepbound and Mounjaro contain the same molecule, but FDA approves uses by brand and by evidence. Zepbound’s label lists two uses, weight reduction and sleep apnea in people with obesity, and no cardiovascular one.[4] SURPASS-CVOT enrolled only people with type 2 diabetes, so its result does not transfer to Zepbound’s label automatically. How Zepbound itself works on weight is covered in how Zepbound works.
The trial that could change that is SURMOUNT-MMO, which enrolled 15,374 adults with obesity and compares tirzepatide with placebo. Its primary outcome is a broader composite of death from any cause, heart attack, stroke, coronary revascularization and heart failure events. ClinicalTrials.gov lists it as active and not recruiting, with primary completion estimated for October 2027.[5] Any Zepbound heart indication would have to wait for those results and an FDA review.
Among weight-loss drugs, Wegovy (semaglutide) is the one with a heart indication for people carrying excess weight who already have cardiovascular disease. In its SELECT trial, the hazard ratio for heart attack, stroke and cardiovascular death against placebo was 0.80.[8] That trial is summarized at SELECT.
What this means for people taking Mounjaro
- If you have type 2 diabetes and heart or artery disease, Mounjaro now has label-backed evidence on heart events, the same kind of evidence Trulicity has.
- The trial does not show that switching from Trulicity to Mounjaro lowers heart risk further. It showed tirzepatide was at least as protective, with a trend in its favor that did not reach significance.[1]
- Mounjaro was added on top of usual heart care. Most participants stayed on statins, blood pressure drugs and antiplatelet drugs, and investigators were told to keep treating lipids and blood pressure to target.[2] How a GLP-1 drug fits with cholesterol treatment is covered in statins and a GLP-1.
- Insurance coverage depends on the indication your doctor bills under and on your plan’s formulary, not on the label alone.
