Kisspeptin: What the Human Studies Show, and What They Don't
Kisspeptin, the hypothalamic hormone that drives GnRH and LH, works in hospital studies as a hypogonadism test and an IVF trigger. It has no weight-loss trials, no FDA approval, and kisspeptin-10 is in FDA's Category 2 for safety risks.
Kisspeptin is a natural hormone made in the hypothalamus that switches on the release of GnRH, which in turn drives the pituitary hormones LH and FSH and, downstream, testosterone and ovulation. The human evidence is real but narrow. In hospital studies, a single dose works as a diagnostic test for hypothalamic hypogonadism and delayed puberty,[10][12] triggers egg maturation in IVF,[13][14] and changes how the brain responds to sexual images.[21][24] It has never been tested as a weight-loss drug, and it did not change appetite or food intake in people.[25][26] Kisspeptin is not approved by FDA for any use, and kisspeptin-10, the form sold online, has been on FDA’s list of compounding substances that raise significant safety risks since September 2023.[1][2]
What kisspeptin is
Kisspeptin was first found by cancer researchers. In 2001 a Japanese team showed that a gene called KiSS-1, known to suppress the spread of melanoma and breast cancer, encodes a 54-amino-acid peptide. They named it metastin.[5] Its importance for reproduction emerged two years later, when two groups studying families whose children never went through puberty traced the condition to broken copies of the peptide’s receptor, then called GPR54.[6][7] People without a working receptor have hypogonadotropic hypogonadism: the brain never sends the signal that starts the reproductive system.
That is the core of kisspeptin’s job. Kisspeptin neurons in the hypothalamus fire in pulses, which set the rhythm of GnRH release, which sets the rhythm of LH and FSH from the pituitary, which drives the testes and ovaries.[28] Kisspeptin sits one step above GnRH. The body makes several lengths of the peptide. Kisspeptin-54 is the main form in human blood and the one used in most clinical studies. Kisspeptin-10 is the short fragment that carries the active end, and it is the version most often sold online.[9][13] Kisspeptin is also made in the placenta, liver, pancreas, fat tissue and parts of the brain that handle emotion, which is why researchers have looked beyond reproduction.[27]

What kisspeptin does to hormones
The first human test came in 2005. Six men received a 90-minute infusion of kisspeptin-54 and, on another day, saline. Average LH more than doubled, from 4.2 to 10.8 units per liter, and testosterone rose from 21.7 to 24.9 nmol/L. The peptide’s half-life in blood was about 28 minutes.[8] A 2015 study that compared kisspeptin-10, kisspeptin-54 and GnRH head to head found the two kisspeptin forms behaved much the same, and GnRH produced two to three times as much LH as either.[9] Kisspeptin is a gentler, more upstream push on the same system.
The catch for anyone injecting it daily is that the system stops responding. Women with hypothalamic amenorrhea who received kisspeptin-54 injections twice a day saw LH jump by 24 units on the first day. By day 14 the rise was 2.5 units.[17] The same thing happened in a 2026 study of healthy men given kisspeptin-10 under the skin by continuous pump: after five days, LH and FSH were back to placebo levels. Only an intermittent schedule, eight hours on and sixteen off, kept LH modestly raised for 12 days.[18] Those were carefully timed infusions with blood checks. A fixed daily injection from a vendor protocol has no such testing behind it.
Kisspeptin as a test for hypogonadism and delayed puberty
This is where kisspeptin has its clearest use. Doctors can test the pituitary directly by giving GnRH, but there is no standard test of the hypothalamus above it.[28] Kisspeptin fills that gap, because it only works if the GnRH-producing neurons are there and functional.
- Adult men. In 21 men with congenital hypogonadotropic hypogonadism and 21 healthy men, a single dose of kisspeptin-54 raised LH by 12.5 units in healthy men and 0.4 in the men with the condition. Every man with the condition rose by less than 2 units and every healthy man by more than 4, a perfect separation. GnRH did less well at the same task.[10]
- Children with delayed puberty. In the first pediatric study, children with delayed or stalled puberty split into clear responders and non-responders.[11] When the Boston team followed 16 of them, all eight who had responded later went through puberty, and all eight who had not reached age 18 without any signs of it. The kisspeptin test predicted the outcome better than the GnRH test, inhibin B or genetic testing.[12]
These are small studies from specialist centers, and the test is not an approved diagnostic product. It is a strong research result, and it says nothing about whether kisspeptin treats low testosterone.
Kisspeptin as an IVF trigger
In IVF, a final “trigger” injection makes the eggs mature before collection. The usual trigger, hCG, carries a risk of ovarian hyperstimulation syndrome, a fluid shift that can be dangerous. Kisspeptin causes the body’s own LH surge instead, which wears off faster.
- First test, 2014. In 53 women given a single injection of kisspeptin-54, eggs matured at every dose tried. Embryos were transferred in 92%, and 23% had a clinical pregnancy.[13]
- High-risk women, 2015. In a phase 2 trial of 60 women at high risk of hyperstimulation, eggs matured in 95%. Per transfer, the live birth rate was 45%. No woman developed moderate, severe or critical hyperstimulation syndrome.[14]
- A second dose, 2017. In 62 high-risk women, adding a second injection 10 hours later raised the share with a good egg yield from 45% to 71%, without more hyperstimulation.[15]
- Compared with other triggers. In a look back at one clinic’s high-risk patients, the odds of a hyperstimulation diagnosis were 33.6 times higher after hCG and 3.6 times higher after a GnRH agonist trigger than after kisspeptin.[16]
These trials came from a single London IVF unit, and the last comparison was retrospective, not randomized. The authors themselves called for head-to-head trials against established triggers.[15] Kisspeptin is not an approved IVF drug in the US. Newer work tests an experimental kisspeptin-receptor drug, MVT-602, whose LH rise lasts far longer than kisspeptin-54’s,[20] and a nasal spray of kisspeptin-54 that raised LH in early testing without reported side effects.[19] Both remain in research.
Kisspeptin, libido and the brain
Kisspeptin’s reputation as a libido peptide comes from brain-scan studies. In 2017, 29 healthy young men lay in an MRI scanner during a kisspeptin infusion and, on another day, a placebo infusion. Kisspeptin increased activity in emotion and reward areas when the men looked at sexual and couple-bonding images, and it reduced negative mood.[21] A 2020 study found it boosted brain responses to attractive faces and scents, most in men with lower sexual quality-of-life scores.[22]
The same team then ran two crossover trials in people with hypoactive sexual desire disorder (HSDD), each comparing one 75-minute infusion with one placebo infusion:
- Women. Of 40 premenopausal women randomized, 32 completed both visits. Kisspeptin changed activity in several brain regions during erotic videos and pictures of faces. The main outcomes were scan measures, not desire or sexual activity. No side effects were reported.[23]
- Men. Of 37 men randomized, 32 completed. Kisspeptin changed activity in sexual-processing brain areas, increased penile tumescence while watching sexual videos by up to 56% more than placebo, and raised a questionnaire score for “happiness about sex” by 0.63 points.[24]
These are promising signals, and they come from a lab session, not from life. Each person got one infusion by vein. Nobody took kisspeptin at home, for weeks, or by injection under the skin, and nobody measured whether desire or sex changed over time. Compare bremelanotide, sold as Vyleesi: it went through two 24-week phase 3 trials before approval for low desire in premenopausal women, and the gain over placebo was still small. That record is set out in our PT-141 evidence review. Kisspeptin has not started that path.
Kisspeptin, appetite and weight: no evidence
Some sellers list fat loss or metabolism among kisspeptin’s benefits. The rodent data are mixed: some studies found it reduced food intake and others found no effect.[26] In people, the answer so far is no effect on eating.
- Lean men. In 15 healthy men, a kisspeptin infusion made the pancreas release more insulin after a glucose load given by vein. It did not change gut hormones, appetite or food intake.[25]
- Women with overweight or obesity. In 17 women with an average BMI of 34, a kisspeptin infusion raised LH, confirming it was active, but did not change hunger or how much they ate: 6.8 against 6.2 kcal per kg of body weight on placebo, a difference that was not significant. Glucose and insulin did not change either.[26]
No trial has given kisspeptin to people to see whether they lose weight. A search of ClinicalTrials.gov in October 2026 for studies using kisspeptin as an intervention returned 36 entries. They cover hypogonadism, puberty, IVF, amenorrhea, polycystic ovary syndrome and insulin secretion. None tests weight loss.[29] The only human eating data are two single-session studies, the larger in 17 people.
Is kisspeptin legal? FDA status
No kisspeptin product is approved by FDA. FDA’s drug-approval database and the DailyMed label library return no match for it. That leaves compounding. A US pharmacy compounding under section 503A may use an ingredient only if it has an official monograph, is part of an approved drug, or is on FDA’s 503A bulks list.[3] The list in federal regulation names six substances, none of them a peptide.[4]
FDA keeps a holding system for nominated ingredients while it decides. Kisspeptin-10 was nominated, and on September 29, 2023, FDA placed it in Category 2 for 503A pharmacies: substances that raise significant safety risks.[1][2] For Category 2, FDA says it does not extend the tolerance it gives Category 1 substances and “would consider taking action” against a compounder that uses one.[3] FDA’s stated reasons:
“Compounded drugs containing Kisspeptin-10 may pose risk for immunogenicity for certain routes of administration and may have complexities with regard to peptide-related impurities and API characterization. FDA has no, or only limited, safety-related information for the proposed routes of administration. Therefore, the agency lacks sufficient information to know whether the drug would cause harm when administered to humans.”[1]— FDA, Certain Bulk Drug Substances for Use in Compounding that May Present Significant Safety Risks
| Question | Answer |
|---|---|
| Is any kisspeptin product FDA-approved? | No. No kisspeptin product appears in FDA’s approval database or on DailyMed. |
| Is it on the 503A bulks list? | No. The list names six substances, none a peptide.[4] |
| Where does kisspeptin-10 sit on FDA’s category lists? | 503A Category 2, significant safety risks, since September 29, 2023.[1][2] |
| Is it the withdrawn-nomination kind of Category 2? | No. It is in the active Category 2 table, not the “nominated but withdrawn” table.[1] |
| Was it on the July 2026 advisory committee agenda? | No. That meeting covered BPC-157, KPV, TB-500, MOTS-c, emideltide, Semax and epitalon.[32] |
| Is it allowed in sport? | No, for male athletes. WADA lists kisspeptin and its agonists as testosterone-stimulating peptides in males, prohibited at all times.[30] |
Our peptide legal-status guide sets kisspeptin-10 beside the other peptides on the same lists, and 503A vs 503B pharmacies explains the two compounding channels. Gonadorelin, which is GnRH itself rather than its upstream trigger, has a different history: it once held FDA approval for diagnostic and fertility use, covered on our gonadorelin page.
What sellers claim, against the trial record
Kisspeptin-10 is usually sold online in vials labeled “for research use only.” Marketing tends to promise higher testosterone, stronger libido, better fertility and sometimes fat loss. Here is how each claim compares with the studies above.
| Claim | What the studies show |
|---|---|
| Raises testosterone | A single hospital infusion raised testosterone modestly in men.[8] Repeated or continuous dosing blunted the hormone response.[17][18] No trial has treated low testosterone with it. |
| Boosts libido | Single-infusion brain-scan trials; one found more penile tumescence in men with HSDD.[24] No take-home or long-term trial. |
| Improves fertility | Real phase 2 IVF data, using kisspeptin-54 as a one-time trigger under specialist care.[14] Not a fertility tonic. |
| Burns fat or curbs appetite | No effect on appetite or food intake in two human studies; no weight-loss trial exists.[25][26][29] |
| Safe because it is natural | FDA says it lacks the information to know whether compounded kisspeptin-10 causes harm.[1] |
The form matters too. Most of the clinical record uses kisspeptin-54, given by vein, dosed per kilogram, in a research unit with blood tests. The product sold online is usually kisspeptin-10 for self-injection under the skin. The two forms raised LH similarly when infused,[9] but that does not make a vendor vial equivalent to a study drug. The “research use only” label does not change the law either. In a December 2024 warning letter to a peptide seller, FDA said that despite “RESEARCH USE ONLY” labels, the seller’s websites showed its products were “intended to be drugs for human use.”[31] That letter concerned GLP-1 products; the same issues are covered in buying tirzepatide as a research chemical.
Side effects and safety
In the published hospital studies, single doses were well tolerated. The HSDD trial in women reported no adverse effects, the nasal-spray study reported none, and in the obesity study heart rate and blood pressure did not change.[23][19][26] That is reassuring for one supervised dose of a pharmaceutical-grade peptide. It is not a safety record for months of self-injected kisspeptin-10 of unknown purity. FDA’s concerns are about exactly that: immune reactions to the injected peptide, impurities, and how well the ingredient is characterized.[1] Anyone with a hormone-sensitive condition, who is pregnant or trying to conceive, or who is on testosterone or fertility treatment should treat kisspeptin as an unstudied drug acting on the same system.
Kisspeptin has more human studies behind it than most peptides on clinic menus, which our peptide menu review counts for the others. Very few of its studies tested it as a treatment people take for weeks. For a peptide sold for weight loss with no human data at all, see MOTS-c. For what is known about GLP-1 drugs, testosterone and male fertility, see GLP-1s and men’s testosterone.
