Does Cigna Cover Zepbound and Wegovy? Express Scripts Covers the Pen, Not the KwikPen
Cigna says weight-loss drugs are excluded under many of its plans. Where covered, Express Scripts' 2026 National Preferred Formulary lists the Zepbound single-dose pen and Wegovy, and excludes Zepbound KwikPens and vials. Cigna's 2026 criteria and options if denied.
Cigna can cover Zepbound and Wegovy for weight loss, but only if your employer added weight-loss drugs to your plan, and only for certain forms of the drug. Cigna’s own policies say weight-loss medications “are specifically excluded under many benefit plans.”[6] Where the benefit exists, Express Scripts, the pharmacy manager inside Cigna’s Evernorth business, lists the Zepbound single-dose pen and Wegovy pens, Wegovy HD and Wegovy tablets as covered on its 2026 National Preferred Formulary, while the Zepbound KwikPen and Zepbound vials are excluded.[1] Prior authorization is required, with criteria Cigna updated in June and September 2026.[4][5] For type 2 diabetes, Cigna approves Mounjaro and Ozempic under its own diabetes policy.[7]
The Zepbound you get depends on the form
Zepbound comes in three forms: a single-dose pen, a multi-dose KwikPen, and single-dose vials. Express Scripts treats them differently.
| Product | Status | Note |
|---|---|---|
| Zepbound single-dose pen | Covered option | Prior authorization required |
| Zepbound KwikPen | Excluded | Covered alternatives listed instead |
| Zepbound vials | Excluded | Excluded since January 1, 2026 |
| Wegovy pens and Wegovy HD | Covered option | Prior authorization required |
| Wegovy tablets | Covered option | Listed among the covered alternatives |
| Foundayo | Covered option | Listed among the covered alternatives |
The exclusion row lists liraglutide, Foundayo, Wegovy HD, Wegovy pens, Wegovy tablets and Zepbound pens as the covered alternatives to the excluded KwikPens and vials.[1][2] Express Scripts’ change list for January 2026 shows that the vials and Saxenda were new exclusions that year, with Wegovy and the Zepbound pen named as the replacements.[3] If your prescription says KwikPen or vial, ask your prescriber to switch it to the single-dose pen before you file a claim, or the pharmacy will reject it as non-formulary.
The split lines up with how Lilly sells Zepbound for cash. Lilly’s self-pay prices apply to the KwikPen and the vial, not to the single-dose pen.[13] For a Cigna member that means two separate routes: the pen through Express Scripts if your plan covers it, or the KwikPen or vial at Lilly’s cash price if it does not.
Not every Cigna plan uses the National Preferred Formulary, and Express Scripts notes that not every listed drug is covered by every plan.[2] Some employers keep their own list. The drug list named in your benefits booklet is the one to check.
Whether your employer covers weight loss comes first
Cigna writes the same sentence into its weight-loss drug policies: weight-loss medications are excluded under many plans, both employer groups and individual and family plans, and the plan document decides whether the benefit exists.[6] If your plan excludes the category, a prior authorization for weight loss is not the issue; the plan simply does not pay for the drug class. Some Blue Cross plans go further and exclude Wegovy even when it is prescribed for heart disease. Ask HR or Cigna two questions: does my plan include anti-obesity medication coverage, and which Express Scripts formulary do we use?
Employers that do cover these drugs can also buy cost programs from Evernorth. In July 2025 Express Scripts said its EncircleRx program would cap the monthly copay for GLP‑1 weight-loss drugs at $200, and that its Patient Assurance Program would add Wegovy and Zepbound with a copay of no more than $25 for up to a 34-day supply.[9] Both are company-reported, both apply only where a health plan or employer participates, and the announcement gave no start date for the $25 copay, so ask whether your plan is enrolled before counting on either figure.
Cigna’s prior authorization criteria for weight loss
Cigna’s Zepbound policy took effect September 1, 2026, and its Wegovy injection policy was last reviewed June 17, 2026. For adults, the two read almost identically.[4][5]
- Three months of diet and behavior change first. The patient needs a documented three-month attempt at changing eating habits and behavior, continued once the drug starts. That is half the six months Aetna asks for.[4]
- A starting BMI of 30 or more, or 27 or more with at least one of a long list of conditions: high blood pressure, type 2 diabetes, abnormal cholesterol, sleep apnea, heart or coronary artery disease, knee osteoarthritis, asthma, COPD, fatty liver disease or polycystic ovary syndrome. The BMI that counts is the one from before any GLP‑1 drug.[4]
- Eight months, then 5%. The first approval runs eight months. Renewal, for a year at a time, needs at least 5% loss from the starting weight.[4][5]
- Wegovy HD needs a step. Cigna treats the 7.2 mg dose as a next step for adults who have been on Wegovy 2.4 mg without trouble for a month or more and still need to lose more.[5]
Cigna also limits retail fills: across all GLP‑1 drugs combined, it approves one claim for a supply of 28 days or more every 21 days.[8] That stops overlapping fills when someone switches drugs. If a request is denied, our guide to prior authorization and appeals covers the deadlines.
Zepbound for sleep apnea
Of the two, only Zepbound carries an FDA approval for sleep apnea, limited to obstructive apnea that is moderate or severe in people with obesity.[10][11] In SURMOUNT‑OSA, tirzepatide cut breathing interruptions by 20 to 24 events an hour more than placebo after a year.[12] Cigna approves Zepbound for sleep apnea for a full year at a time when the patient is an adult with a starting BMI of 30 or more and a sleep study showing an apnea-hypopnea index or respiratory event index of at least 15 events an hour (Kaiser’s Northwest region asks for 30), without central sleep apnea or Cheyne-Stokes breathing.[4] After the first year, renewal needs at least 10% weight loss and a prescriber’s statement that symptoms such as snoring and daytime sleepiness have stayed stable.[4] Cigna’s policy does not ask for a failed trial of CPAP, which UnitedHealthcare’s sleep apnea program does. Our article on Zepbound for sleep apnea covers the evidence.
Wegovy for heart risk and MASH
Cigna covers Wegovy for one year at a time as heart protection for adults whose starting BMI was at least 27 and who have had a prior heart attack, a prior stroke (a transient ischemic attack does not count) or symptomatic peripheral artery disease, when the prescriber confirms the drug will be used alongside optimized heart medication.[5] The criteria set no minimum age above 18 and do not exclude people with diabetes.[5]
For MASH, Cigna asks for stage F2 or F3 fibrosis without cirrhosis, documented by a liver biopsy within the previous three years or by a FibroScan, MR elastography or ELF blood test within the previous six months, plus metabolic risk factors being managed.[5]
Mounjaro and Ozempic for type 2 diabetes
Both are on Express Scripts’ 2026 National Preferred Formulary, along with the new Ozempic tablets.[1] Cigna’s diabetes policy, reviewed June 3, 2026, approves them for a year when the patient has type 2 diabetes: Ozempic from age 18, Mounjaro from age 10. The same policy lists weight loss as a use it does not cover, because neither drug is approved for it.[7]
If Cigna will not cover it
Start with the form. A rejected KwikPen or vial claim may only need a new prescription for the pen. If the plan excludes weight-loss drugs, check whether sleep apnea, heart disease or MASH applies to you, since those requests rest on a different diagnosis. Cigna members on Medicare should look at the federal Medicare GLP‑1 Bridge instead.
If none of that works, the cash prices are the floor. Lilly sells the Zepbound KwikPen and vial for $299 at 2.5 mg, $399 at 5 mg, and $449 at higher doses when refilled on time.[13] Novo Nordisk sells Wegovy for $349 a month at most doses, and its savings card can bring an insured fill to $25 when the plan covers the drug.[14] Our ranking of the cheapest GLP‑1 without insurance compares every option.
