Epitalon

Also known as Epithalon, Ala-Glu-Asp-Gly, AEDG peptide

A four-amino-acid peptide sold as a telomere and anti-aging treatment. The telomere results are from cell cultures, and the famous mortality figures belong to a cattle pineal extract, a different substance.

Regulatory status
No FDA approval and not on the 503A Bulks List. Since October 28, 2026 an interim HHS policy puts state-licensed pharmacies and physicians outside federal enforcement when they compound epitalon acetate under section 503A as a subcutaneous or intramuscular injection dispensed in intermittent courses, single-ingredient, from tested bulk peptide out of an FDA-registered facility, on a patient-specific prescription. Capsules, sprays and research-use-only vials fall outside it. It approves nothing and lasts until a final rule settles the listing.

Overview

Epitalon, also spelled epithalon, is a four-link synthetic peptide with the sequence Ala-Glu-Asp-Gly. Researchers in St. Petersburg built it from the amino-acid makeup of epithalamin, an extract of cattle pineal glands [7]. It is sold as an anti-aging peptide on the claim that it switches on telomerase, the enzyme that rebuilds the protective caps at the ends of chromosomes.

The extract and the synthetic peptide are easy to confuse, and sellers often do. The best-known human results, lower death rates among elderly people followed for years, come from epithalamin. FDA treats the two as different substances [8]. For epitalon itself FDA found three small human studies, none designed to show a health benefit, and no clinical data on its safety [8].

On October 28, 2026 an interim HHS policy opened a narrow compounding route. State-licensed pharmacies and physicians may compound epitalon acetate as an injection given in intermittent courses, under conditions, without expecting enforcement action [9]. The policy does not approve it and does not touch the evidence.

How it works

The core finding is from cell culture. In 2003 the St. Petersburg group added the peptide to human fetal lung fibroblasts that do not normally make telomerase and reported that the cells began producing it and their telomeres lengthened [1]. A follow-up reported that treated cells went on dividing for ten passages beyond the point where untreated cells stopped [2].

In 2025 a lab at Brunel University London, outside that group, saw telomeres lengthen in normal human cell lines through telomerase. In breast cancer cell lines the telomeres also lengthened, through a separate mechanism called ALT [3]. That second result matters for safety as much as for efficacy, since unlimited cell division is a feature of cancer. FDA made the same point in its review: by activating telomerase the peptide could in principle raise cancer risk, and the animal studies were too limited to rule that out [8].

What the evidence says

Animal work is mixed. In a 2003 study of 54 mice per group, monthly injections did not change average life span, total tumor rates or body weight. The longest-lived tenth of treated mice lived 13.3% longer than their counterparts, maximum life span was 12.3% longer, and leukemia was less common [4].

In people, the headline numbers are not about this peptide. A 2003 paper reported 1.6 to 1.8 times lower mortality over six to eight years in elderly patients given epithalamin [5], and a 12-year study reported 28% fewer deaths in the epithalamin group than in controls [6]. Both used the pineal extract. The three human studies of epitalon FDA identified covered gene expression and a melatonin marker in night-shift workers and an eye disease, retinitis pigmentosa, in small groups [8]. An orphan drug designation for retinitis pigmentosa was granted in 2010 and revoked in 2016 [8].

Almost all of the literature comes from one research group, and the only independent replication so far is in cell lines. Grade D. Our evidence review sets out each study, the source of the mortality figures and what FDA's 2026 review found.

Typical dosing

No human dose has been established. The mouse study used about 30 to 40 mcg per kg for five days each month [4], and the human studies of the peptide itself were too small and too different in route to set a regimen. Vendor and forum dosing has no tested basis.

The HHS policy shapes how a compounded product may be given rather than how much. It covers only subcutaneous or intramuscular injection of epitalon acetate, dispensed in intermittent courses, as the sole active ingredient, on a prescription for one patient [9]. Capsules and oral sprays, which FDA found on sale, fall outside it [8].

Safety & side effects

There is too little data to describe the risks in people. FDA found no acute, repeat-dose or reproductive toxicity studies, its adverse-event searches returned no reports, and it concluded that no clinical data support the peptide's safety in humans [8]. HHS, explaining its policy, wrote that the few people given it under controlled conditions had no reported side effects, and that people take it in short courses rather than continuously [9]. Both statements rest on very few people.

The theoretical concern is cancer. A compound that lengthens telomeres, and did so in cancer cells in one lab, deserves long-term study before anyone takes it for years, and that study has not been done [3] [8]. Under the October 28, 2026 policy a covered pharmacy must source tested bulk peptide from an FDA-registered facility and tell FDA through MedWatch about serious harm, which at least starts a record [9]. HHS can withdraw the interim policy if a safety problem surfaces. Research-use-only vials come with none of those checks.

Frequently asked questions

Is epitalon the same as epithalamin?

No. Epithalamin is a mixture of peptides extracted from cattle pineal glands; epitalon is a single synthetic four-amino-acid peptide modeled on it. FDA treats them as different substances, and the human mortality studies used epithalamin.

Does epitalon lengthen telomeres in people?

That has not been shown. Telomere lengthening was seen in human cells grown in the lab, first by the Russian group that developed the peptide and in 2025 by a British lab. No study has measured it in people taking epitalon.

Can a pharmacy compound epitalon now?

Since October 28, 2026, an interim HHS policy puts state-licensed pharmacies and physicians outside federal enforcement when they compound epitalon acetate under section 503A as an injection dispensed in intermittent courses, on a patient-specific prescription and under quality and reporting conditions. It is not FDA-approved and not on the 503A Bulks List.

Is it safe?

Unknown. FDA found no clinical safety data and no standard toxicity studies, and flagged a theoretical cancer risk because the peptide activates telomerase. The small human studies reported no side effects, but they enrolled too few people to tell.

Are epitalon capsules or sprays covered by the new policy?

No. The policy covers only subcutaneous or intramuscular injection of epitalon acetate. Oral products remain outside it.
