Where Each Medication Stands[8]
| Medication | What Anthem does |
|---|---|
| Wegovy (semaglutide) | Coverage varies by Anthem-licensed plan and state. Elevance Health (Anthem's parent) added Wegovy for the cardiovascular-risk-reduction indication on supported plans. Effective January 1, 2026, Anthem Medi-Cal removed Wegovy, Zepbound, and Saxenda from coverage when prescribed for weight loss; PA requests are still considered for non-weight-loss indications (Wegovy for MASH or established CVD, Zepbound for obstructive sleep apnea). |
| Ozempic (semaglutide) | Covered for type 2 diabetes diagnosis in most Anthem plans. |
| Zepbound (tirzepatide) | Varies by Anthem plan. Anthem Medi-Cal excludes Zepbound for weight loss as of Jan 1, 2026 but considers PA for OSA. Virginia Anthem Medicaid criteria require BMI >40, or BMI >37 with risk factors (dyslipidemia, hypertension). |
| Mounjaro (tirzepatide) | Covered for type 2 diabetes; step therapy may apply. |
| Copay when it is covered | $25-$80 / month |
After a Denial from Anthem
A refusal on Wegovy or Zepbound is not the end of the road. Licensed 503A pharmacies[7] dispense compounded semaglutide and tirzepatide at $149-$299 a month, well under what the brand costs. Two other things are worth knowing. With a prescription, a brand-name GLP-1 is generally an eligible HSA or FSA medical expense[5], and a good number of plan administrators will accept a compounded one on a Letter of Medical Necessity. The services listed below ship to all 50 states and usually write inside the week.
Large employers often self-fund, which puts the plan under ERISA rather than state insurance law and leaves the employer free to decide whether anti-obesity medication is covered at all[6]. If yours self-funds, the answer sits in the Summary Plan Description, in the GLP-1 language specifically.
Lodge the formal appeal with Anthem first. Ask your prescriber for a Letter of Medical Necessity that documents your BMI, any comorbidities, and the weight-loss attempts that did not work. Appeals carrying one overturn a real share of denials.
Anthem publishes its criteria, and our guide walks all of them: the Virginia Medicaid threshold of BMI > 40, the steepest in the commercial PA triplet; the Medi-Cal exclusion California applies from 01/01/2026; the CC-0188 framework for weight management, which wants a BMI of 30 or more, or 27 alongside a comorbidity, plus a behavioral program lasting 3 months; the way CarelonRx as PBM applies the Wegovy-preferred formulary step; how far 14 BCBS subsidiaries diverge from one another; and the CMS Medicare GLP-1 Bridge, which starts 07/01/2026. Read it in the Anthem GLP-1 Prior Authorization Guide.
Cash-pay GLP-1 services if Anthem says no
Cash-pay compounded semaglutide and tirzepatide are not hard to find, and after a denial the monthly cost often lands under what a covered copay would have been. Our highest-ranked picks come first, then the longer list.
