What is Dose-based pricing?

A plan whose price climbs as your dose is titrated up.

The advertised figure is the starting dose, and the cost of a full course is a different number entirely. The opposite arrangement is flat pricing. Which one a seller uses matters more to a year's spend than the headline rate does.
